General Manager, Direct-to-Patient Extension Health / EHRX — New York City (In Person) | Full-Time OTE $180K - $230K About Extension Health & EHRX Extension Health is NYC's premier longevity medical center, an all-under-one-roof platform for personalized healthspan optimization, offering the most advanced diagnostics and advanced therapeutics available today. Founded by Dr. Jonathann Kuo, MD, our physician-driven team has helped 50,000+ patients take control of their health and longevity across our West Village and Midtown flagship locations. EHRX is our fast-growing telehealth brand, bringing compounded prescription peptide therapy directly to patients nationwide. LegitScript Certified, 503A pharmacy-backed, and physician-led, EHRX is building a rigorous, accessible model for direct-to-patient care. We're building something rare: a clinically credible brand with serious digital infrastructure. This is a significant leadership opportunity to own the direct-to-patient business across both brands. The Role We're looking for a commercially driven General Manager to own and grow our direct-to-patient business across Extension Health and EHRX. This is a true GM role. You'll hold full P&L responsibility, set the revenue strategy, and build the team and systems needed to scale. You are not a functional specialist. You're a builder and an operator who knows how to drive revenue, move fast, and make smart decisions with imperfect information. You understand that the best direct-to-patient health businesses win on trust, product, and conversion, and you know how to optimize all three. This role reports directly to the CMO / VP of Revenue/Growth and works closely with the clinical and marketing teams. What You'll Own • Full P&L responsibility for direct-to-patient revenue across Extension Health and EHRX, with clear quarterly and annual targets, • Revenue strategy and execution, from traffic acquisition and conversion rate optimization to retention, LTV, and subscription growth, • EHRX growth, scaling the telehealth business nationally, including channel strategy, patient acquisition, funnel optimization, and new treatment launches, • Extension Health online pharmacy, optimizing our online pharmacy for patients and members, • Go-to-market leadership, owning product launches, promotional calendars, and pricing strategy for EHRX, • Paid media and performance marketing, either managing directly or overseeing an agency/internal team across Meta, Google, and emerging channels, • Technology and operations, owning the direct-to-patient stack, conversion funnel, and telehealth patient journey from first click to first shipment, • Team building, hiring and developing a high-performance team as the business scales, • Reporting and accountability, weekly revenue reporting, forecasting, and clear-eyed analysis of what's working and what isn't What We're Looking For • 6-10 years of experience in DTC, digital health, or direct-to-patient businesses, with at least 2-3 years in a senior leadership or GM-level role, • A track record of hitting revenue targets and growing an online business, not just managing one, • Experience with DTC performance marketing. You understand CAC, LTV, payback periods, and how to build a profitable acquisition engine, • Familiarity with telehealth, healthcare ecommerce, or regulated consumer health products is a strong plus, • Operator mentality. You build systems, hold people accountable, and stay close to the numbers, • Comfortable in a founder-led, fast-moving environment where the strategy evolves and execution has to keep pace, • Strong commercial instincts. You think about margin, pricing, and unit economics, not just top-line growth, • Excellent communicator and cross-functional collaborator.
