Job Description Job description: Accura Health is seeking an experienced Full-Time Field Nurse Practitioner to provide high-quality primary and post-acute care services to complex patients in the home setting. This role focuses on Transitional Care Management (TCM) and House Calls, with an emphasis on preventing avoidable hospitalizations and improving patient outcomes.The Nurse Practitioner will manage an assigned coverage area and work collaboratively with physicians, care teams, and internal clinical staff. Key Responsibilities Clinical Care & Patient Management • Conduct in-home evaluations and follow-up visits for complex post-acute patients, • Perform comprehensive routine and urgent assessments, • Document findings, treatments, and care plans accurately and timely, • Provide physical and emotional support to patients and families during periods of illness or crisis, • Communicate clinical findings and care recommendations with the Medical Director, ordering physicians, care team, and internal staff, • Coordinate all aspects of patient care across disciplines, • Order, perform, and interpret diagnostic tests (laboratory and radiology) within scope of practice, • Prescribe medications, including controlled substances, per licensure and collaborative agreements, • Order treatments and durable medical equipment (DME) as clinically indicated, • Maintain accurate, compliant patient records to support quality care and reimbursement, • Ensure compliance with all applicable laws, regulations, and collaborative practice agreements, • Initiate and support reimbursement processes for services rendered, • Assist with training, mentoring, and supervision of nursing trainees and advanced practice students, • Participate in development and refinement of clinical practice guidelines, • Engage in ongoing professional development and continuing education, • Supervise nursing trainees, graduate nurses, and advanced practice students during field placements, • Certified Nurse Practitioner (NP), • Active Texas Nurse Practitioner license in good standing, • Minimum 2 years of Nurse Practitioner experience, • Minimum 2 years of Primary Care experience, • Valid CPR certification, • Valid Texas driver’s license, auto insurance, and reliable transportation, • Experience in post-acute care, geriatrics, home health, or transitional care, • Primary Care, • Geriatrics, • Home Health, • Internal Medicine, • Wound Care, • Pediatrics (as applicable)
