Responsible for coordinating safe patient transitions, ensuring medical necessity, reviewing level of care, discharge planning, and payer authorization processes. Works closely with interdisciplinary teams and post-acute services. Required Skills • Acute Hospital Case Management / Utilization Review, • Care Coordination & Plan of Care development, • Admission Criteria Review & Determine Medical Necessity, • Discharge Planning & Needs Assessment / DME orders, • Prior Authorizations, Appeals & Denials, • Worker’s Comp Case Management (preferred) Regulatory & Technical Knowledge • CMS, DRG, HIPAA, Joint Commission / Core Measures, • Documentation & payer compliance standards Clinical Experience Experience in any of the following acute settings: At 3B Healthcare, we empower hospitals and healthcare organizations with sustainable workforce solutions.
