This 255-unit residence will provide housing for 153 individuals and families referred by the New York City HIV/AIDS Services Administration (HASA) and the rest 102 units will be apartments for moderate-income households. The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population. Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Midtown Community Justice is seeking a Bilingual Case Manager for the Intake team. Reporting to the Coordinator of Court Operations, the Case Manager will be responsible for coordinating new referrals, conducting intakes and screenings, reengaging former and existing participants to complete mandated services, orienting new participants to the Midtown Community Justice Center, and scheduling participants for services. The Case Manager will also be responsible for providing one-on-one case management support to participants in need of services.
