Job Description Care Coordinator Bedford-Stuyvesant Restoration Corporation, founded in 1967, is the nation’s first community development corporation nestled in the heart of Central Brooklyn in Bed-Stuy. Located at Restoration Plaza, a 300,000 square foot commercial complex that offers a mix of retail, dining, recreation and entertainment, this historic organization is steeped in its mission to close the racial wealth gap. We provide direct services across an economic mobility platform centered on workforce development, affordable housing/homeownership, community revitalization, small business development and arts and culture to 60,000 people annually. About the Role The Care Coordinator will provide holistic and comprehensive case management services to all participants, ensuring access to wraparound supports that promote financial stability, employment, and overall well-being. Key Responsibilities • Conduct intake triage and assessments across domains including benefits, financial health, education, employment, and social environment., • Complete Financial Health Assessments and PHQ-9 screenings (for depression, anxiety, stress, and substance use) as part of the initial evaluation process., • Develop individualized service and goal plans to connect clients with appropriate services, including benefits enrollment, financial counseling, education and training, and job placement or retention support., • Discuss and identify barriers to asset building and financial independence, such as unemployment, lack of childcare, limited education, or housing instability, and make referrals to internal and external resources., • Conduct intake and triage sessions with 100% of newly assigned participants and determine appropriate next steps to help them achieve both short- and long-term goals., • Ensure 100% of unemployed or underemployed clients are referred and scheduled to meet with a Career Development Specialist., • Ensure 100% of participants are scheduled for Financial Counseling sessions., • Document all participant interactions, progress updates, and outcomes in accordance with organizational and funder requirements., • Record and follow up on declined referrals, using motivational interviewing to support behavior change and engagement., • Inform clients of relevant community events, workshops, and programs (e.g., Restoration Outreach events)., • Follow up on all referrals and document outcomes, maintaining thorough case notes that reflect participant progress., • Meet all contractual obligations and program performance benchmarks., • Utilize Restoration’s Database, Salesforce, and Google Suite applications (Docs, Drive, Mail, Sheets) for accurate data entry and case tracking., • Perform other duties as assigned. What you bring to the role • Education: Bachelor’s Degree with 2–3 years of social service experience preferred., • Experience with benefits eligibility screening (e.g., SNAP, health insurance, SCRIE, DRIE) is a plus., • Knowledge of community resources and counseling or social work practices serving underserved populations (e.g., unemployed individuals, low-income families, or those at risk for homelessness)., • Strong communication (written and verbal) and documentation skills., • Ability to establish rapport and motivate clients toward goal achievement., • Demonstrated ability to work independently and manage multiple priorities in a fast-paced environment., • Strong confidentiality, integrity, and professional boundaries., • Work occasional eveings and weekend hours as needed., • Bilingual ( Spanish/ English) preferred but not required., • Cultural competence and ability to work effectively with diverse families and communities. $24.73- $27.47 What We Offer: Full Time: Medical, Dental and Vision Insurance, Vision plan paid for you and your family, 403B retirement benefits, 12 paid holidays, a generous PTO bank of 4 weeks, paid sick leave, Employee Discount Program, Employee Assistance Program, Commuter Benefits programs, and other forms of leave and benefits.
