Job DescriptionDescription: QRM is seeking a part time Telehealth PT, for a PRN or part time position (approx 5-10 hours/week). Candidates must be licensed in multiple states. The Telehealth Rehabilitation Associate is a member of the Quality Improvement and Education (QIE) team who provides therapy services to patients remotely. The Telehealth Rehabilitation Associate reports to the Director of Quality Improvement and Education. Key Results Area • Supervise Therapist Assistants and therapy students in direct patient care to carry out plan of care and patient-related activities per the state practice act and reimbursement regulations, • Provide high-quality, patient-centered therapy evaluations, treatments, and discharges via telehealth platforms, • Deliver skilled interventions aligned with Medicare and payer regulations, SNF policies, and professional practice standards, • Document timely, accurate, and comprehensive therapy notes in the electronic medical record (EMR) in accordance with regulatory and professional guidelines, • Engage patients through interactive, functional therapy activities adapted for virtual platforms, • Support facility staff through caregiver education, recommendations for equipment or environmental modifications, and interdisciplinary coordination, • Adhere to all telehealth best practices, HIPAA regulations, and facility protocols for remote service delivery, • Participate in case conferences, care planning meetings, and team communication as appropriate, • Demonstrate a positive attitude and a collaborative team-building approach with all patients, clients, and team members, • Attend QIE educational webinars as requested, • Respond to email communication within four business hours and follow up by the deadline, • Evaluate and assess personal performance on a routine basis, • Licensed Physical therapist (Compact Licensure preferred) with a minimum of 5 years of experience in LTC. Responsible for maintaining license requirements in all states they are providing services, • Proficiency in federal and state reimbursement, regulatory and quality programs, • Independent and self-motivated, • Ability to provide information in a concise manner, • Collaborative and detail-oriented professional with strong analytical and organizational skills, • Strong proactive written and verbal communication supporting client satisfaction and retention, • Strong critical thinking and clinical decision-making skills, • Strong time management with a proven ability to meet deadlines, • Function well in a fast-paced and at times stressful environment, • Demonstrate discretion, sensitivity, and ability to maintain confidentiality, • Ability to establish and maintain positive relationships, • Function independently, have flexibility, personal integrity, and the ability to work effectively with patients, clients, and team members, • Microsoft Office and EMR proficiency, with the aptitude to learn new programs

As a Milieu Coordinator at ERC Pathlight, you can expect to be a part of team that delivers hands-on treatment, group therapy sessions and/or support to our patients in one of our many centers around the nation. Our Program Therapists are responsible for facilitating therapeutic groups based on core curriculum. The Milieu Coordinator offers milieu support, de-escalation and behavior coaching to patients in IP/Res and PHP programs. You will build on your education in this therapist in training role! How you'll serve our patients: A Day In The Life As a Primary Therapist - Youtube Video As a Primary Therapist at ERC Pathlight, you can expect to be a part of team that delivers hands-on treatment, therapy sessions and/or support to our patients in one of our many centers around the nation, or virtually through our tele-behavioral health platform. The Primary Therapists Conducts individual, family, and group therapy (adults, children and adolescents) on topics such as mood, anxiety, stress and eating disorders. Our Primary Therapists develop each patient's individualized treatment plan and work with interdisciplinary treatment team to implement the plan. You will serve patients daily in a multitude of ways: * Conduct psychotherapy including individual, group and family therapy * Conduct comprehensive assessments and treatment planning for each patient on case load: * Complete biopsychosocial, self-harm and suicide assessments * Create individualized treatment plan with specific and measurable goals in collaboration with treatment team members * Create treatment agreements, support plans or behavior plans as needed * For each patient on case load, complete clinical paperwork including: * Treatment plans * Individual and Family Therapy progress notes and group notes * Wellness plan and aftercare plan * Discharge summary * Attend treatment team meetings and treatment team rounds to coordinate patients care with interdisciplinary team Primary Therapist I Must haves: • Master's or Doctorate Degree required in counseling or related field, • Registered as an associate or intern phase of licensure or what is required by the state to practice individual therapy, • Experience with individual/family therapy preferred Primary Therapist II Must Haves: * Has obtained and maintains a full clinical mental health license as required by state * Has carried significant individual and/or family caseload for at least 2 years with documented success in treating patients with common diagnoses and expected comorbidities Primary Therapist III Must Haves: * At least 3 years post licensure experience working with mood, anxiety, ED and/or trauma related disorders * Some experience supervising other clinicians * Has carried a significant individual and/or family caseload, with success treating more complex cases What We Offer At ERC Pathlight, caring for people is our mission and that begins with caring for our own team.
