Job Description Position Summary: The Community Based Nurse is responsible for providing health care supports, training and technical assistance for persons diagnosed with intellectual/ developmental disabilities (I/DD). The community Based Nurse will work with agency nurses, health care coordinators, supports coordinators and the community health care systems. The Community Based Nurse works within the interdisciplinary team to ensure optimum quality care. Considering the financial availability, PCHC will pay for attendance to the National Developmental Disabilities Nurses Association (DDNA) conference. Duties and Responsibilities: Nursing: • Participate in the ODP Health Risk Screening Tool (HRST) Quality Module, • Complete Community Health Reviews, Transition Reviews or other nursing assessments as requested., • Participate in Team Meetings, Provider, County and Regional Meetings as needed, • Create and present live and virtual trainings on various nursing topics, • Clinical consultant for individuals with complex health conditions, • Act as a resource regarding medical/ dental treatment, care and follow up, • Complete Nursing Reviews (Community Health Review, Transition Review, etc.), • Work in partnership with agency nurses to assist individuals and their teams through health care issues, • Establish alliances with other nurses and health coordinators who are working in the community with individuals who have been diagnosed with I/DD, • Remain active in updating skills and use best practice in working with people with I/DD, • Participate in meetings, committees, attend training seminars as required, • Participate in Nurse Network and other health related meetings, at the local, regional and state level, • Work in conjunction with other PCHC nurses and all other PCHC staff members, • Provide training in physical in physical and behavioral health care issues for individuals diagnosed with I/DD, staff members who support them, family members, supports coordinators, and any other supportive personnel, • Identify staff training needs as they relate to health and safety. Provide training on physical and behavioral health care issues utilizing existing training resources and developing training modules, as necessary., • Be a resource to individuals diagnosed with I/DD, agencies, counties, and the state/ regional Office of Developmental Programs/ Pennsylvania Department of Public Welfare for issues involving health care, • Ability to work independently, • Excellent communication skills, both written and oral as well as organizational skills, • Ability to provide training in health-related matters, • Ability to establish and maintain positive working relationships with co-workers and other disciplines outside PCHC, • Registered Nurse, • Valid Pennsylvania Nursing License, • Two year's experience in working with individuals diagnosed with I/DD preferred (other acceptable nursing experiences include public health nursing, community health nursing, behavioral health nursing.) This member of the ACT multidisciplinary team is responsible for overseeing a consumer caseload of individuals and provide rehabilitation services, including symptoms management, life skills development, vocational support, educational support, socialization support and budgeting/money management skills. POSITION SUMMARY The Behavior Consultant (BSC) performs assessments of a child’s, youth’s or young adult’s needs and development of an individualized treatment plan (ITP) that includes the interventions that will be used to meet the identified needs. The BSC also coordinates and consults with other service providers regarding behavior plans and interventions and discussion with the child’s, youth’s or young adult’s treatment team about the behavioral interventions that should be used and when they should be used. BSC services can be used to address behaviors in the school, home, or any other place where a child, youth or young adult needs behavior support (i.e.- across settings) and to learn strategies to better succeed. POSITION RESPONSIBILITIES • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented across settings of the child., • Develop a trusting partnership with family members and other team members., • Ensure clinical interventions are properly formulated in order to design, implement, monitor and adjust the behavioral interventions for the child to ensure progress can be made over time and in accordance with the ITP., • Observe child in the home/school/community setting as part of the initial assessment in preparation for designing the individualized treatment plan and implementation of the written order. Functional Behavior Assessments (FBA) will also be needed (if properly trained with certificate)., • Design data collection tools to be used by Behavior Health Technician (BHT) and/or other team members to accurately assess effectiveness of established therapeutic interventions., • Develop Individualized Treatment Plans (ITP) to instruct other members of the therapeutic team (including teacher, teacher’s aides, other school personnel, family, BHT, MT and/or other designated individuals providing direct service or supports to the child) in their roles and responsibilities in implementing the established interventions., • Complete and submit accurate and timely documentation which includes but is not limited to: Individualized Treatment Plans (ITP), Encounter Forms, Progress Notes, Progress Summaries, Addendums, Crisis/Safety Plans, FBAs, Assessments, Structured Tools, Incident Reports, ITM summaries, Discharge Summaries, etc. by assigned deadlines., • Provide individual mentoring and supervision of all assigned BHT staff, • Conduct Functional Behavior Assessments (if properly trained with certificate) –FBA for BSC-ASD/BC-A requires a professional license plus FBA certification from DPW or a professional license plus BCBA. For non ASD FBA’s – the FBA certification is sufficient., • Supervision Requirements:, • BSC clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides BSC services supervises BHT treatment team members, the BSC must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: BSC clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • BSC's must complete at least 40 hours of training within the first year and 20 hours each year thereafter. These training hours must be Department-approved and related to the specific job functions and in accordance with their individual training plan. The training hours will also include mandatory agency and/or funder or state requirements., • Valid Act 34, Act 151 and Act 114 clearances. EDUCATION/CREDENTIALS/EXPERIENCE As taken from Annex A, Title 55: Human Services, Part III: Medical Assistance Manual, Chapter 1155: Intensive Behavior Health Services General Provisions, individuals who provide individual services through BSC must meet one of the following: • Be licensed in this Commonwealth as a behavior specialist., • Have a certification as a BCBA or other graduate-level certification in behavior analysis that is accredited by the National Commission for Certifying Agencies or the American National Standards Institute., • Have a graduate degree in ABA from a college or university accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation or have an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. or the National Association of Credential Evaluation Services. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency., • Have a minimum of 1 year of full-time experience in providing mental health direct services to children, youth or young adults and a graduate degree in psychology, social work, education, or counseling from a college or university accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation or an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. or the National Association of Credential Evaluation Services. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency., • Completed a clinical or mental health direct service practicum and have a graduate degree in psychology, social work, education, counseling or a related field from a college or university accredited by an agency recognized by the United States Department of Education or the Council for Higher Education Accreditation or have an equivalent degree from a foreign college or university that has been evaluated by the Association of International Credential Evaluators, Inc. or the National Association of Credential Evaluation Services. The Department will accept a general equivalency report from the listed evaluator agencies to verify a foreign degree or its equivalency. Additionally, in order to provide behavior consultation to children diagnosed with Autism Spectrum Disorder (ASD) for the treatment of ASD, the BSC shall be licensed in this Commonwealth as a psychologist, professional counselor, marriage and family therapist, clinical social worker, social worker, behavior specialist, certified registered nurse practitioner or a professional with a scope of practice that includes overseeing the provision of ABA services. This is a full-time salaried position. The Lead Behavior Consultant carries out these functions by directing and supporting the Behavioral Consultants and Behavioral Health Technicians in delivering superior service. The Lead Behavior Consultant is responsible for monitoring the census and staffing for their programs to insure the program is meeting all requirements. The Lead BC is a liaison with the school team and the IBHS team, in addition to all other teams involved with assigned clients. The Lead BC will complete Functional Behavior Assessments and Group Mobile Therapy sessions for children in the assigned schools. The Lead BC shall provide clinical direction of services for each child on their caseload, shall revise and update the Individualized Treatment Plan (ITP), and shall provide oversight of the provision of the ITP. The Lead BC should consult with adults across settings in that child’s life and create appropriate and reasonable goals. Supervision should address Interventions, Progress toward ITP goals, Adjustments to ITP and Skill in implementing the interventions., • Provide weekly on-site supervision and support to Behavioral Health Technicians., • Provide 6 hours of on-site supervision to any new BHTs after they can work alone., • Provide 1 hour of direct-observation supervision every 4 months to address Interventions, Progress toward ITP goals, Adjustments to ITP, • Assists in implementation of program procedures., • Must bill at least 15 hours of services each week, • Provides child-centered, strengths-based group psychotherapy weekly for all clients at the site., • For any unstaffed cases, due to call outs or shortages, provides child-centered, strengths-based individual psychotherapy on a weekly basis until coverage resumes. This could take the form of the Behavior Consultant or the Behavioral Health Technician position. The LBHT carries out specific behavioral management interventions identified in the treatment plan for the child(ren) or adolescent(s) assigned to their care. This role involves the provision of 1:1 service with a child or adolescent. Services may also be provided to integrate or reintegrate the child into normalizing settings and activities. The LBHT is responsible for providing behavioral health interventions to stabilize the child, assist the child in developing age-appropriate self-management skills, and to support, and empower parents, family members, teachers or other adults responsible for supervising the child in an educational or other community setting. The LBHT works with the child, school staff, family and the entire treatment team by implementing the plan techniques in order to achieve the desired goals. The LBHT also acts as a mentor to other BHT team members and assists them with acclimation to the role and the school. The LBHT, in conjunction with the Lead BC, acts as a liaison between the IBHS program and the school. II.
