Job Description The Sr. Evaluation Analyst will support CBH performance evaluation activities by producing both routine and ad hoc data analyses and reports associated with CBH’s quality initiatives defined by OMHSAS, including Performance Improvement Plan (PIP) and Program Evaluation Performance (PEP) efforts as well as National Committee for Quality Assurance (NCQA) and other Quality Management initiatives. The Sr. Evaluation Analyst will provide critical analyses and interpretation of data and will present executive summary reports of these analyses to CBH senior leadership and contribute to the narrative of CBH’s State reporting requirements. This position reports to the Supervisor of Data Analytics in CBH’s Data Analytics department, under the Operations division, with the majority of the incumbent’s work supporting the evaluation and reporting needs of CBH’s Quality Management Performance Evaluation department. Job Description When you work as a RBT Trained Clinician with Access Services, you'll be working to access fieldwork hours towards clinical requirements as you deliver behavior support services across child, adolescent, and adult populations. This dual-role position of RBT Trained Clinician involves providing clinical direction, developing treatment plans, and conducting behavioral assessments for adults, while also implementing individualized behavioral interventions for children and adolescents under Intensive Behavioral Health Services (IBHS). Responsibilities Elwyn seeks a Care Coordinator to coordinate and manage care across the spectrum of healthcare services, including access to high-quality physical health (e.g. acute and chronic,) and behavioral health care, as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and the recovery of the whole person. The Care Coordinator identifies specific needs early to develop concrete, resolution-focused plans to support the child and family in the most appropriate, least restrictive way. This role ensures seamless care coordination for individuals referred to and/or returning from a higher level of care such as hospitals, inpatient units, or residential settings. DUTIES AND RESPONSIBILITIES: • Work directly with families and schools, gathering information including but not limited to individual education plans and prior behavioral health history, to determine possible services needed for the child(ren), • Complete screening tools with the families to identify the social determinants that may be barriers to accessing or engaging in behavioral health services, including but not limited to One Care Vermont: Self-Sufficiency Outcomes Matrix and the Cognitive Behavior Intervention, In Schools (CBITS), Screener for Anxiety Related Disorders (SCARED), child outcome survey directly with the child(ren) and families, • Provide coordination of all services for IBHS consumers and their families well as linkages to new services needed; including but not limited to referrals to other levels of care, • Completing insurance packets prior to original authorization, for every authorization cycle or as needed based on client's treatment needs in a timely manner. IBHS authorization packets will be submitted to MCO's within 24 hours of receiving, • Tracking and monitoring authorization dates in accordance with CBH and MCO's standards; communicating with interagency team members in a timely manner about due dates and documents needed. Authorize and extend treatment authorizations when Medical Necessity Criteria is met., • Execute discharge plan by making all referrals to community resources, • Learn the inner workings of the assigned schools by taking part in ongoing school meetings to be able to demonstrate understanding of the Multi-Tiered System of Supports (MTSS) and understand when/how to intervene to build relationships with school personnel and navigate complicated school systems, • Act as the liaison between the assigned school, provider, and families, attending ongoing school meetings related to each child's academic and behavioral performance to identify, link, and coordinate services according to the service plan created by the treatment team, • Work collaboratively with the treatment team including school staff family engagement specialists, Student Assistance Program (SAP) assessors, and other identified district behavioral health staff to create better educational and behavioral outcomes for the child, • Monitor the effectiveness of service delivery by participating in treatment team progress summary meetings, hearing and providing behavioral progress/updates on the children, • Re-assess services to prevent the need for more restrictive or acute care services; coordinate IBHS service termination and referral to recommended services, if needed, • Perform cardiopulmonary resuscitation (CPR), first aid and crisis intervention using agency-trained protocols, • Perform other duties as assignedEDUCATION/SKILLS/EXPERIENCE REQUIREMENTS:, • Bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, or education, PLUS one year experience (paid or unpaid) in a human service field. This experience must involve direct contact with the individual receiving services (i.e. coaching, teaching, case management, etc.) OR be a registered nurse, OR have a high school diploma and twelve (12) semester credit hours in sociology, social welfare, psychology, gerontology, or other social science PLUS two years paid experience in public or private human services with one year in direct client contact, • Ability to keep pace with active children, • Experience in navigating the behavioral health and other service systems preferred, • Experience with providing care coordination in a health care and or human service field for individuals requiring access and linkage to needed resources, • Experience in navigating social service systems e.g. those related to physical health, education, employment, housing, insurance, etc. preferred, • Demonstrated ability to work effectively as part of a team, • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities, • Demonstrated strong attention to detail, • Demonstrated strong time management and organizational skills, • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision, • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions, • Must be able to perform CPR, first aid and crisis intervention using agency-trained protocols, • Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills, • Demonstrated basic experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint, • Knowledge of Electronic Health Record (EHR) / Electronic Medical Record (EMR) preferred, • Must possess a current, valid driver's license in state of residence, have a minimum of two (2) years of driving experience within the United States, and have an acceptable driving record
