Responsibilities Elwyn seeks a Care Coordinator to coordinate and manage care across the spectrum of healthcare services, including access to high-quality physical health (e.g. acute and chronic,) and behavioral health care, as well as social services, housing, educational systems, and employment opportunities as necessary to facilitate wellness and the recovery of the whole person. The Care Coordinator identifies specific needs early to develop concrete, resolution-focused plans to support the child and family in the most appropriate, least restrictive way. This role ensures seamless care coordination for individuals referred to and/or returning from a higher level of care such as hospitals, inpatient units, or residential settings. DUTIES AND RESPONSIBILITIES: • Work directly with families and schools, gathering information including but not limited to individual education plans and prior behavioral health history, to determine possible services needed for the child(ren), • Complete screening tools with the families to identify the social determinants that may be barriers to accessing or engaging in behavioral health services, including but not limited to One Care Vermont: Self-Sufficiency Outcomes Matrix and the Cognitive Behavior Intervention, In Schools (CBITS), Screener for Anxiety Related Disorders (SCARED), child outcome survey directly with the child(ren) and families, • Provide coordination of all services for IBHS consumers and their families well as linkages to new services needed; including but not limited to referrals to other levels of care, • Completing insurance packets prior to original authorization, for every authorization cycle or as needed based on client's treatment needs in a timely manner. IBHS authorization packets will be submitted to MCO's within 24 hours of receiving, • Tracking and monitoring authorization dates in accordance with CBH and MCO's standards; communicating with interagency team members in a timely manner about due dates and documents needed. Authorize and extend treatment authorizations when Medical Necessity Criteria is met., • Execute discharge plan by making all referrals to community resources, • Learn the inner workings of the assigned schools by taking part in ongoing school meetings to be able to demonstrate understanding of the Multi-Tiered System of Supports (MTSS) and understand when/how to intervene to build relationships with school personnel and navigate complicated school systems, • Act as the liaison between the assigned school, provider, and families, attending ongoing school meetings related to each child's academic and behavioral performance to identify, link, and coordinate services according to the service plan created by the treatment team, • Work collaboratively with the treatment team including school staff family engagement specialists, Student Assistance Program (SAP) assessors, and other identified district behavioral health staff to create better educational and behavioral outcomes for the child, • Monitor the effectiveness of service delivery by participating in treatment team progress summary meetings, hearing and providing behavioral progress/updates on the children, • Re-assess services to prevent the need for more restrictive or acute care services; coordinate IBHS service termination and referral to recommended services, if needed, • Perform cardiopulmonary resuscitation (CPR), first aid and crisis intervention using agency-trained protocols, • Perform other duties as assignedEDUCATION/SKILLS/EXPERIENCE REQUIREMENTS:, • Bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, or education, PLUS one year experience (paid or unpaid) in a human service field. Job Description Position Summary: The Project Manager for the Behavioral Health and Justice Division (BHJD) is responsible for designing, piloting, and proposing the sustainability of all new and expanding initiatives created by BHJD though grants, reinvestment opportunities, or existing funding. The Project Manager will oversee the development and implementation of strategic plans to connect the work of each new initiative to the overall mission and vision of BHJD and DBHIDS. The Project Manager will continually work to create new stakeholder relationships as well as strengthen communication between BHJD and project partners involved in all new initiative creation. The Project Manager will work closely with the Business Operations and Finance Manager to secure grant funding or allocate existing funds for all new projects while ensuring efficient business practices and equitably between new and existing initiatives. Duties and Responsibilities: Design, pilot, and propose sustainability plans for all new and expanded initiatives created by BHJD through grants, reinvestment opportunities, or existing funding. Job DescriptionThe IBHS Clinical Care Manager – Neurodevelopmental Services (CCM-ND) is responsible for utilization management by conducting utilization review activities, population management, cross-system consultation, provider management and treatment quality monitoring. The Clinical Care Manager must utilize clinical knowledge of best practices and evidence-based treatments, including Applied Behavior Analysis, for individuals with Autism spectrum and other neurodevelopmental disorders, to ensure CBH members receive timely access to quality, medically necessary behavioral health services. When clinically indicated, Clinical Care Managers consult with CBH Physician and Psychologist Advisors to ensure the most effective treatment for the CBH member is being authorized. Job Description We are seeking a Rehabilitation Counselor to work in our Behavioral Health Services Division. To provide support, education, counseling, and guidance to children or adolescents who have been identified as having mental health or developmental disabilities. Assist with client movement, goal achievement, and local community interaction. Implement the overall mission, values, and beliefs of Horizon House in all aspects of the job. Other Responsibilities, but not all-inclusive: · Assist with the growth and development of the child/adolescent and the development of therapeutic support skills. · Provide one-on-one behavioral intervention in the home, school, or community settings in accordance with strategies identified in the Treatment Plan. · Participate in developing goals with the client for inclusion in the individualized service plan. ·Ensure provision of preventive healthcare and necessary follow-up services for clients. Qualifications: · Bachelor’s Degree in Human Services/Social Services, Healthcare, or related field. At least 1 year of experience working directly with children; or H.S. Diploma or GED equivalent plus 12 behavioral health college credit hours from an approved educational program, plus a minimum of 2 years of experience (paid or unpaid) in a human service field.
