Job DescriptionSalary: $40,000 - $50,000 Case Manager Job Description Job Title: Case Manager Department: Restorative Care Reports To: Restorative Care Director FLSA Status: Full-time (Exempt) Organization Elijah Rising is a Christian non-profit organization that provides safety through Prayer, Awareness, Intervention, and Restoration for victims of sex trafficking. Elijah Rising's Restoration program serves survivors through residential care in a therapeutic setting. Centered around the eight dimensions of wellness, Elijah Rising provides safe housing, food, utilities, clothing, personal supplies, access to medical and dental care, professional therapy, life skills training, emotional skills, occupational resources, job skills, case management, spiritual care, and transportation 24/7/365. Summary The case manager (C.M.) plays a key role in the Residents lives as they are a constant, stable presence and therefore must be a role model and a good listener. Good communication skills, resourcefulness, attention to detail, time management skills, and administrative skills are mandatory. As an integral part of the Elijah Rising Restoration Program team, the C.M. oversees the trajectory of the pathway through the program with each resident. This role is a part of the CARE Team and works closely with the Restorative Care Director to assess and resource each client to have her needs met. The case manager is responsible for working with each unique resident to map out practical steps to reach her goals, which could include aiding her with medical needs, public assistance, educational goals, completion of program work, personal improvement goals, and more. This requires someone who has a gift for patiently teaching new concepts with compassion, best social work practices, and accountability with a trauma-informed approach. Aside from direct case work, key duties include developing key program components, tracking metrics, managing each case with proficiency and structure, transportation, collaborating as a leader on the program team, managing the program Point System, activities, and appointments on a daily basis. Required Duties and Responsibilities (Other duties may be assigned as needed) • Leads and guides Bible studies, devotionals, and prayer and worship times as needed., • Assist in intake of new residents by reviewing referral records and applications, • Perform a needs assessment of every new resident, • Develop individual service plans by analyzing needs assessment and pairing the individuals with the necessary treatment care, • Empower the residents to progressively meet their needs independently, • Maintain residents records by reviewing case notes, logging service plans, service provision, incidents, and progress, • Draft MOUs with preferred providers to ensure the best possible care for the residents., • Develop, maintain, and refresh a resource database and files to support a wide range of client needs, • Determine the key needs of each resident, coordinating with the residential team to have needs met, • Transport residents to appointments as needed and takes appropriate notes for documentation, • Oversee medication documentation with Residential Manager (R.M.) and communicate needs or changes based on doctors guidance, • Updates resident files after each case management meeting or important conversation, • Executes all case management for residents, • Continuously develops and improves the standardized case management process, • Aids in conflict resolution by facilitating healthy communication between residents, taking proper notes, and reporting additional support needed to the Program Director, • Must have excellent people skills with the goal of developing trust with staff and residents in order to assist them with effective care, • Must be detail-oriented in order to effectively track and report metrics, • Must be able to balance programming with administration to meet all deadlines, • Excellent computer skills are needed, • Able to create spreadsheets, graphs, charts, presentations, and other documents, • Must be able to manage time efficiently to meet expected weekly deadlines, • Able to read, analyze, and interpret professional journals and general business periodicals, • Able to write reports, presentations, blogs, essays, as needed, • Able to speak publicly and present to different groups as needed, • Must be able to work with mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations, • Able to define problems, collect data, establish facts, and draw valid conclusions, • Proficient in Google Workspace tools, • Comfortable navigating Apple/macOS systems, • Knowledge of 1Password for secure credential management, • Working knowledge of BambooHR, • Effective communication using Slack or similar internal messaging tools, • Must have reliable transportation, • Must possess a valid state drivers license and valid vehicle insurance, • Must be available to oversee and ensure staff coverage for 24/7 care as needed during emergency situations, • Must be able to drive a 15-passenger van, • Must be able to drive on the highway for long distances (2 hours+), • Must successfully pass background checks and random drug testing during employment, • Employees are frequently required to stand, walk, and drive, • Vision requirements include close vision, far-range vision, and the ability to focus, • Hearing requirements include the ability to hear normal conversation in normal ranges and the ability to be in moderate to loud environments for extended periods of time, • Occasionally required to lift/carry and push/pull up to 50 lbs Education, Training, and Experience • Bachelors Degree preferred, • One or more years of social/victim services experiences, preferably trauma-informed required, • Great computer skills and the ability to learn client management software quickly
