The Center for Family Representation Inc. (CFR) is an award-winning, innovative law and policy organization that serves primarily Black and Brown families and youth in Manhattan, Queens, the Bronx, and Staten Island with an interdisciplinary legal defense model that serves more than 2400 clients every year. CFR was originally founded in 2002 to dramatically change the trajectory of indigent parents being prosecuted by the City’s Administration for Children’s Services (ACS). Our goal is to defend and protect our clients’ rights, reduce the harm of family separation and prevent or minimize the time any child spends in the foster system. CFR provides interdisciplinary family defense with a model that was unique at our founding and that is now being replicated nationally: we assign every client an attorney and a social work staff member, and teams have the support of parent advocates, who are parents with direct personal experience of having been investigated and prosecuted by ACS. CFR was the first agency in the country to integrate parents with lived experience into legal teams and now has 11 parent advocates working in all four of our family defense locations. CFR’s Early Defense Practice represents parents during an ACS investigation (to avoid court involvement where possible). Our SCR Practice assists parents in clearing their names from state maltreatment records. Certified Recovery Peer Advocate services are peer-delivered services with a rehabilitation and recovery focus. Job Description About the Role Hamilton-Madison House (HMH) is the leading provider of behavioral health services for New York City’s Asian community and the second largest provider in the country. HMH serves nearly 1,000 community members through a comprehensive program model that understands and incorporates the strengths of individual cultures and addresses the stigma related to mental illness. Services range from psychological services; personalized recovery-oriented services for individuals with mental illness; recovery programming for addiction in six languages and seven dialects; supportive housing for people with mental health issues; and training and internship opportunities for graduate students in social work, mental health counseling, and psychology. Key Responsibilities • Conduct initial and ongoing housing needs assessments with clients., • Develop individualized housing stabilization and retention plans that promote long-term stability and self-sufficiency., • Assist clients in preparing and submitting housing applications, including the 2010E form, and provide ongoing support through the placement process., • Provide coaching and support to help clients maintain their households, meet lease obligations, manage finances, and prevent eviction., • Intervene in crisis situations and employ mitigation strategies to preserve tenancy and client safety., • Identify and link clients to ongoing medical, behavioral health, and community-based support as part of a coordinated care approach., • Participate actively in case conferences, supervision, and team meetings., • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values., • Other duties as assigned. Requirements: QUALIFICATIONS • High School Diploma or Equivalency required; college degree and/or CASAC credential strongly preferred., • Minimum of 2 years of experience working with individuals affected by substance use, mental illness, or homelessness., • At least 1 year of experience in housing placement, advocacy, or case management., • Strong engagement and communication skills with diverse populations., • Ability to navigate social and healthcare systems and coordinate across multiple providers., • Proficiency with data entry and electronic documentation systems., • Knowledge of housing resources, subsidies, and support programs available in New York City, particularly Staten Island., • Bilingual (English/Spanish) strongly preferred.

The Pyramid Safe Haven is a 125-bed, low-barrier shelter operated by BronxWorks for chronically street-homeless single adult men. Located in the Melrose section of the Bronx, Pyramid follows a Housing First model and provides on-site services including case management, psychiatric and medical care, substance use counseling, meals, showers, and housing placement support. Referrals are made exclusively through NYC’s Street Homeless Outreach Teams. The program focuses on building trust and engagement while supporting clients in transitioning into stable, long-term housing. RESPONSIBILITIES • Independently manage a caseload, • Conduct comprehensive intake assessments., • Conduct regular field visits to engage and build rapport with chronically homeless individuals., • Work with clients to develop goals and service plans., • Provide referrals and direct assistance to support clients applying and/or attending appointments, • for mental health services, medical care, substance treatment, vital documents, benefits,, • legal/immigration services, and permanent housing., • Document all interactions with or on behalf of clients, monitor and document client progress, • toward service plan goals., • Maintain familiarity with social services and resources available both on-site and in the, • community., • Coordinate client care with other service providers; attend regular case conferences with, • clients and other service providers, as required., • Attend regular supervision sessions with direct supervisor., • Drive agency vehicle to and from client locations and appointments, • Perform additional duties as assigned. QUALIFICATIONS: • Bachelor’s degree in social work or a related field of study, • Candidate must have a valid driver’s license which has been held for at least 1 year; in the last 3 years, candidate must have obtained no more than 3 license violation points, • One-year of case management or related work experience preferred., • Ability to engage and build rapport with populations with high support needs., • Strong oral and written communication skills, • Detail-oriented and resourceful with strong time management and organizational skills, • Proficiency in Microsoft Office suite and other standard business technology •, • Bilingual/multilingual preferred, but not required., • Valid driver’s license is required for this position.
