Job DescriptionDescription: General Description The Residential Case Manager (RCM)’s core function is the coordination of care for resident services, providing direct services to residents and assisting in the day-to day activities of running the Luce St. Community Residence household. Essential Duties and Responsibilities • Provides a wide array of rehabilitative and supportive services, which focus on intensive, goal-oriented intervention to address issues identified by and specific to a resident’s needs regarding community integration or goal-oriented interventions which focus on improving or maintaining skills to enable a resident to transition to a more independent housing setting., • Is responsible for the documentation, development, and/or implementation of the following: assessments, service plan goals, service plan reviews, hospitalizations, discharge planning, progress notes, and income in accordance with OMH regulations. Documentation includes both written and data entry as required., • Maintains accurate case file records and reports based on the requirements of each funding source/program., • Submits resident files to the SRS within 28-days of an admission for a complete file audit and review, ensuring compliance with program regulations, policy, procedure, or business practice., • Is responsible for the renewal of file documents with expiration dates that include but not limited to; Release of Information (ROI) and/or Consents to Release Information, and Physician Authorization’s., • Meets regularly with the Senior Resident Supervisor (SRS) for a review of file documentation. The RCM is responsible for file documentation in accordance with funding regulations, agency policy, procedure, or business practice., • In collaboration with SCR is responsible for ensuring that the CR is maintained at standards established for recertification; reporting the need for household furniture/supply replacement or property maintenance in accordance with agency policy, procedure, or business practice., • Assists residents, as necessary in complying with recommendations given by their treatment/service providers., • Acts as an advocate/mediator for residents experiencing difficulties with treatment/service provider(s), employment, or funding related issues to ensure that the rights and confidentiality of the resident are not overlooked or denied., • Acts as a resource and assist residents transitioning from the program to a more/less restrictive level of care, ensuring a continuity of care with providers and funding streams., • Conducts routine bedroom inspections to identify those who may need help keeping their rooms clean; communicating concerns to the SRS and working with the resident to maintain their space., • Participates in the maintenance of the home-like atmosphere, including family-style dinners, household chores, in-house activities, shopping, outings, etc., • Attends staff meetings, treatment team/provider meetings, and staff development/training seminars in accordance with agency policy, procedure, or business practice., • Participates in the food pantry services of the CR in accordance with the Food Bank of the Southern Tier. Caseload consists of up to 3 families for 4-6 weeks for crisis services. Services are provided in the family home and community at times convenient for the family. The HISS will complete Intakes, in person, at Elmira Police Station and will provide staffing oversight and support services to clients at the shelter/warming station/motels. Contact at both locations will primarily be with clients who are experiencing homelessness and/or housing crisis. The HISS, will likely encounter clients that have experiences with long term homelessness, mental health, and substance abuse concerns along with other barriers that impact their ability to maintaining suitable/safe housing. In both locations, the HISS will enter client information into the Homeless Management Information System through AWARDS. Job DescriptionDescription: $500 Referral Bonus This position is a brand-new position that will be in our new program in Schuyler County. The Interventionist is a critical member of the HBCI (Home Based Crisis Intervention) team. The Interventionist is responsible for providing treatment to youths aged 5-20 years and their family/caregivers to address the clinical needs of the child and the complex needs of the family unit. The Interventionist will provide crisis intervention and stabilization with the child/youth and family, and case management services such as referrals to other services and ensuring the structural home environment is safe. Interventionists use individualized treatment interventions and evidence-based practices to address identified clinical and family system needs. The Interventionist will carry a caseload of up to 3 recipients. Primary Job Functions • As a member of the HBCI team, provides crisis intervention and intensive case management to youths aged 5 – 20 years to ensure safety and stabilization with the child/youth and caregivers/family in the home, school, and community environments., • Works collaboratively with the HBCI team to respond to referrals and ensure the referred youth meets the criteria for receiving HBCI services as defined in HBCI guidelines provided by the NYS Office of Mental Health., • Carries a caseload of up to 3 families providing 4-6 weeks of intensive intervention to include meetings with the child/youth and parents/guardians 3-5 days per week., • Implements individualized treatment interventions developed collaboratively with the family and based on identified clinical and family system needs., • Works with the team to plan and facilitate group therapy and psychoeducation for parents as well as youths., • Works with the HBCI team to ensure implementation of crisis plan and after care plan is scheduled and agreed upon., • Participates in on-call rotation to ensure that crisis intervention services are available to families 24 hours per day, 7 days per week., • Works within the HBCI team to ensure post-discharge follow-up with families occurs in order to assess progress and to evaluate the program’s success.
