Key Responsibilities • Monitor, track, and close gaps in care across value-based programs., • Conduct outreach and follow-up for high-risk patients to support care compliance., • Review medical records and clinical data to identify required preventive screenings, chronic disease management needs, and overdue health measures., • Collaborate with providers, care coordinators, and the quality team to improve population health metrics and clinical performance outcomes., • Support patient education on chronic disease, preventive care, and care plan adherence., • Assist with the development and implementation of value-based care initiatives, including workflows that improve quality scores and patient engagement., • Participate in quality audits, documentation reviews, and reporting for payor-based value programs., • Active RN or PA license in New Jersey (required)., • Experience with value-based medicine, quality measures, or population health strongly preferred., • Knowledge of HEDIS measures, preventive screenings, chronic disease management, or care coordination is a plus., • Strong communication, assessment, and patient engagement skills., • Ability to work independently, manage follow-ups, and prioritize patient needs.
