Opportunities available in the heart of Dallas We are seeking an energetic Clinical Pharmacist to serve as an integrated clinical leader focused on optimizing medication therapy, improving chronic disease outcomes, and advancing population health across ambulatory services. Working within a Federally Qualified Health Center, the role emphasizes patient-centered, evidence-based care for underserved populations while supporting key initiatives such as HEDIS, UDS, medication adherence, HIV care, 340B, and value-based programs. In close collaboration with providers and care teams, the Clinical Pharmacist enhances medication access and quality of care. The position also participates in Collaborative Practice Agreements, where permitted, to manage drug therapy, support medication adjustments, order protocol-driven labs, provide patient education, and document interventions in the electronic medical record. Location: Dallas, TX (5750 Pineland Dr., Dallas, TX); with additional responsibilities to handle at our Mesquite location Here's a sneak peek at what you'll do: • Provide Comprehensive Medication Management (CMM), including medication reconciliation, therapy optimization, and development of individualized care plans., • Identifyand address medication-related issues such as adherence barriers, side effects, drug interactions, affordability, and therapeutic gaps., • Deliver patient counseling and education on medication use, monitoring, and disease management, with coordinated follow-ups for high-risk patients., • Practice under Collaborative Practice Agreements (CPA) to support medication initiation, adjustment, lab monitoring, and protocol-driven care within regulatory guidelines., • Lead and support chronic disease management (e.g., diabetes, hypertension, asthma/COPD) and drive population health initiatives using data and registries., • Support HIV prevention and treatment efforts, includingPrEP/PEP programs, adherence counseling, and coordination of care and monitoring., • Enhance medication access and affordability through 340B program support, formulary optimization, and coordination with pharmacy operations., • Contribute to quality performance initiatives, including HEDIS, UDS, and value-based care metrics, by closing care gaps and improving outcomes., • Collaborate with interdisciplinary care teams and provide education to clinical staff on medication management, safety, and best practices., • Promote medication safety, regulatory compliance, and risk reduction through monitoring, reporting, and adherence to pharmacy laws and policies., • Conduct patient outreach and education with a focus on health equity, cultural responsiveness, and addressing social determinants of health., • Doctor of Pharmacy (PharmD) degree with an active or eligible Texas pharmacist licenserequired., • Ability to practice in compliance with Texas laws, Board of Pharmacy rules, organizational policies, and Collaborative Practice Agreements., • Current BLS certification (or ability to obtain withinrequiredtimeframe)., • Strong knowledge of medication therapy management, chronic disease pharmacotherapy, medication safety, and patient counseling., • Experience working in ambulatory care, FQHC, population health, or health-system settings preferred., • Preferred certifications include BCPS, BCACP, CDCES, MTM, or HIV-related credentials (e.g., AAHIVP, BCIDP)., • Proficiencywith EMR systems (e.g., eClinicalWorks), data tools, and experience with 340B, quality improvement, and value-based care initiatives preferred., • Strong clinical judgment, communication skills, cultural competency, and ability to manage priorities whilemaintainingcompliance and high-quality patient care., • Experience in a clinical setting with a diverse patient population, • MS Office Suite skills (Outlook, Teams, Word,PowerPointand Excel) At HHM Health, our mission starts with caring for people and that includes you.
