Job Description EMERGENCY MEDICAL TECHNICIAN JOB DESCRIPTION / (EMT) SUMMARY: The Emergency Medical Technician (EMT) is responsible for all aspects of patients’ care within the scope of certification. ESSENTIAL DUTIES AND RESPONSIBILITIES: Accurately completes a pre-inspection and post-inspection sheet prior to and after your tour as per NJ DOH/DOT. Performs a visual inspection of the patient compartment, inside and out, and for cleanliness and damage. Cleans the patient compartment, equipment, and outside of vehicle as needed. Replaces damaged or missing items, noting deficiencies on Usage Report. Presents the signed, dated Vehicle/Equipment Checklist to the Supervisor. Ensures that the ambulance is at all times in compliance with applicable NJ Department of Health Regulations. Directs his/her assistant (EMT) with regard to assistance with patient care and handling, use of emergency warning devices, speed and special driving conditions particular to patient needs. Responds to all calls as directed by Communications in a professional manner and in the time frame assigned. Treats all patients in accordance with NJ Basic Life Support Protocols. Secures patient information and records from the discharge planner or Registered Nurse at the transferring facility. Transports all patients via stretcher. (All psych transports must be on stretcher) Carries all patients to and from the ambulance, NO PATIENTS ARE TO WALK. Carefully monitors all patients during transport. Observes all patient rights with regard to courteous conduct, behavior and appropriate communication. Records changes in patient’s condition during transport. Alters patient care in accordance with changes in the patient’s condition. Takes all patients to their assigned location i.e. medical facility or residence. Patients are never to be left at the door. Promptly and accurately completes Pre-Hospital Reports. Properly completes company paperwork and obtains all necessary signatures for billing purposes. Maintains professional conduct and courteous interaction with all medical facility staff, fellow co-workers, and provides conscientious customer service. Possess valid CPR, Blood borne Pathogens, OSHA, & HazMat Certifications. Must wear company uniforms as mandated in company uniform policy. Job Description:\n\nJOB SUMMARY: The Director of Wellness uses their specialized knowledge to insure the Resident’s care and management needs are being adequately and correctly met and assists the Executive Director, and the Director of Case Management as an additional liaison between Residents, the Residents Legal Representatives, their families, administrative staff, and community-based service providers. Responsibilities include the direct supervision of the Wellness and Reflections Unit; motivation, supervision and in-service training/education of the Resident Service Aides. The Director of Wellness will assure compliance with company policy and the State Health Department’s regulatory requirements for the admission and discharge procedure as well as appropriate documentation requirements. Additional responsibilities include: in-service education on health/wellness related topics and on the spot training to address identified performance concerns and/or improvements. As a member of a multi-disciplinary team the Director of Wellness facilitates development of the Resident’s individual plan of care under the direct supervision through collaboration with the Resident, the Resident’s family or Representative, the Resident’s primary care physician, the property Management Team, and any appropriate third-party providers. Addresses all findings as a result of the review and develops appropriate training strategies to address such findings g. Acts as Resident advocate and assists Administrative staff in their continued service to the Resident i. Visits with each Resident to evaluate their concerns ii. Refers all Resident complaints and/or concerns to the Executive Director for immediate investigation and resolution b. Provides oversight to the admission process of the Resident to include: i. Appropriateness and accuracy of the Resident’s physician evaluation for sign off on same ii. Conducts (or delegates as appropriate) Initial Evaluation of Resident prior to admission into the community and follow-up evaluation as per company policy and State Department of Health regulations; • NJ only - If not licensed as an RN forwards Initial Evaluation to designated Registered Professional Nurse; iii. When indicated, assures completion of Behavioral Health evaluation as per company policy and State Department of Health regulation; iv. Conduct as per company policy and State Department of Health requirements functional evaluations to identify Residents status and/or change in status and to determine the type and level of assistance needed: i. Develop and update each Resident’s service plan(s), evaluation, and other pertinent evaluations at specified intervals and when a change in Residents’ status warrants. • NJ only - If not licensed as an RN forwards all updated evaluations to the designated RN for sign off. ii. If delegated by ED, communicates change in Resident status to the Resident’s Representative and appropriate Administrative/Corporate Business staff; iii. Collaborate with a multidisciplinary team of providers, as well as the Resident and/or Resident’s Representative in the development of the Resident’s Individualized Service Plan on admission, appropriate revision within 30 days thereafter, and every 6 months; iv. Monitor the Wellness and Reflections Communication Logs daily for any unusual occurrences or changes in resident’s status and records and insures interventional strategies appropriately; v. Ensure all corporate policies and procedures and State Department of Health Regulations for Assisted Living Residences, Special Needs Program are followed and maintained; vi. Supervise Wellness and Reflections Unit staff to ensure that Residents receive appropriate care and activity program. vii. Ensure monthly support groups are conducted for families in the Reflections programs e. Coordinate care with the Department Heads, Resident’s physician, and other third-party providers: i. Collaborate with the Director of Case Management to assist residents with referrals to pertinent agencies when needed; i.e., Long Term Care insurance forms, nursing homes, social services agencies, Able Ride, etc. ii. Assist with hospital, skilled nursing facility, rehab centers, et al, with transfers and discharges and liaises external staff and discharge planner to determine suitability of Resident to return to facility: • Conduct off site visits as indicated/requested by ED/ Director of Case Management iii. Act as a liaison with Resident’s personal physician and other healthcare providers. iv. Refer Resident service problems to the appropriate Department Managers and Director of Case Management and Administration as necessary. f. Develop and keep accurate Resident records in compliance with company policy and regulatory rule: i. Collaborate in the development of the Resident service/care plans with Dietary Manager, (nutrition consult where indicated), Lifestyle Director, Director of Case Management, Resident’s primary care physician (and consultant specialists), third party providers (particularly home health) and the Resident’s family/Legal Representative; ii. Enter Wellness and Reflections Notes to include necessary social, recreational, physical, emotional, mental / behavioral health findings, needed and ordered interventions, and eventual outcomes iii. Review visiting and outside physician and provider notes and implement any changes in prescribed management as ordered by physician, g. Assure annual physician evaluations and where indicated behavioral health, pain, cognition, wander, fall, skin integrity evaluations; h. Oversee and document review of Residents who are self-administering medications to ensure continued competency and report all findings to ED, Director of Case Management, Resident’s physicians and Representatives – follow up as needed and indicated; i. Conduct skin observations on Residents, documenting findings to maintain compliance, and report findings for follow up. 3. Staff Education, Development, Supervision and Evaluation: a. At the request of the ED: i. Interview, recommend for hire and promotion, train, supervise, evaluate, discipline, and refer to ED for termination: ii. Supervise care staff as well as providing care to Residents iii. Conduct and/or schedule staff in-services to improve the quality of care by care staff to respond to both individual and global staff identified needs iv. Maintain in-service schedules to meet State Department of Health requirements as well as company policy v. Assure in-service training, competency observations vi. Assure Medication Aides are in compliance with all State Department of Health training, documentation of delegated tasks, and ongoing in-services requirements b. Job Description We are looking for a dynamic and compassionate Case Planner to join our team. In this role you will work towards timely permanency planning for children and youth in foster care. Reports to: Foster Care Supervisor Location: 7 West Burnside Avenue, Bronx, New York 10453 Hours: 35 Hours / Week; Non-Exempt - Position requires in office hours and field visits. Hybrid schedules are based on program needs and leadership discretion. Salary Range: 50k-55K based on relevant experience and education. Major Duties: • Assess and monitor for safety and risk., • Establish immediate contact with families of the children who are placed into our care, and with their foster families., • Conduct required casework contacts with the child, parent, foster parent and collateral contacts (schools, service providers, medical personnel, mental health practitioners, and substance abuse counselors). Make assessment-based referrals as necessary to meet the needs of the child and/or parent., • Visit family and child, as often as mandated and/or necessary to gather social history, to assess family strengths and concerns, and to assess existing problems in relation to the reason for placement., • Coordinate and supervise all visits between the family and child along with separate sibling visits., • Share responsibilities with agency team for the permanency planning of each child., • Locate resources and make referrals to any resource that would provide aid to the family and child., • Evaluate child’s adjustment in foster home and monitor educational and behavioral performance at home and in school., • Monitor foster parents in terms of their ability to meet the needs of the child in their home, and make recommendations regarding future use of the home, in conjunction with the Home finding Department., • Adherence to the KEEP (Keeping Foster and Kinship Parents Supported and Trained), PTC (Parenting Through Change) and YDSC (Youth Development Skills Coaching) manuals., • Make efficient and creative use of training manuals., • Capacity to work within the fidelity guidelines of the KEEP/PTC interventions while delivering the material in the manner that appeals to the needs of individual parents and youth., • Support and counsel foster parents in their efforts to care for the child and include them in the development of the goals and tasks., • Keep case records up to date within agency standards and complete all necessary ACS/Agency documentation including FASPs, Progress notes, Permanency Hearing reports and all other reports necessary to fulfill the function of the work., • Make all court appearances and testimony including permanency hearings., • Participate in Family Team Conferences, including but not limited to the placement preservation, reunification/discharge, goal changes and permanency planning conferences. Attend all necessary meetings, functions, workshop, etc. as required by the agency., • Keep case records up to date within agency standards and complete all necessary ACS/Agency documentation including FASPs, Progress notes, Permanency Hearing reports and all other reports necessary to fulfill the function of the work., • Progress notes are to be completed and submitted preferably within one day of the event but no later than the end of the given week of the event., • FASP and Permanency Hearing Report will be submitted on the internal date for review provided by the agency's compliance department., • Collaborate closely with team members., • Conduct Group facilitation of Keep and PTC groups with parents and foster parents., • Ability to enhance communication among parent group members to promote skill development and social networking., • Review KEEP/PTC feedback and practice strategies with families., • Collaborate closely with Youth Skill Trainers, monitor their interactions with youth., • Documents according to developer’s standards, each interaction and visit., • Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar), and self-care, when applicable, in the department/program to create a non-violent and trauma-sensitive culture., • Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture., • Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment, • Utilize Solution Based Casework in casework practice., • Implement Solution Based Casework model of casework practice within the department/program., • Implement and integrate the principles and procedures of Solution Based Casework: assessment, family and individual level outcomes, action plans and documentation and celebration., • Actively participate in case consultation, continued SBC training and the SBC certification process., • Bachelor’s Degree in related field required;, • MSW or at least one year related experience preferred., • Must be able to establish working relationships with clients in need of counseling and supportive services., • Must be able to effectively engage parents., • Ability to apply theory of change., • Willingness to facilitate groups with parents and foster parents., • Ability to work cooperatively with their agency supervisor(s), KEEP, PTC, and YDSC consultants, ACS and other team members., • Excellent written and verbal skills necessary., • Bilingual English/Spanish preferred., • Some evenings may be required Good Shepherd Services is committed to providing an inclusive and welcoming environment for all members of our staff, participants, volunteers, subcontractors, and vendors.
