Philadelphia Medical director - 250 K The Medical director shall be a physician licensed to practice medicine in the state where the hospice is located. In his/her absence, the medical director shall designate another physician to serve as acting medical director. The Medical director of VITAS fulfills two primary functions, each of which is expected to consume approximately one half of the time and work effort of the position: • The Medical director has overall responsibility for medical direction of the care and treatment of patients and their families rendered by the hospice care team, and shall consult and cooperate with the patient's attending physician. The medical director provides physician direction and guidance to the hospice program, its physician employees, and other staff and volunteers to assure the maintenance of quality standards of care for patients and families., • Quality of Clinical Care, • Assure appropriate evaluation and certification of terminal prognosis of patients., • Assure the quality of initial plans of care., • Assure the team manager in the yearly formal evaluation of the team physician., • Ensure that a physician on-call rotation is established so that there is team physician support available 24 hours a day, 7 and denials from third party insurance and intermediaries (i.e. Medicare), • Ensure that all contracted physicians (team physicians and consulting physicians) are properly credentialed via the VITAS Credentialing process., • Serve on the VITAS Credentialing Committee., • In Wisconsin the medical director also supervises the following functions of the consultant pharmacist:, • Ensure medications are utilized within accepted standards of practice., • Ensure a system is developed and maintained that documents the disposal of controlled drugs., • Community Relations, • Educate community physicians on the principles of palliative medicine., • Provide resource and consultative support to community physicians in palliative medicine., • Attend and present at medical staff and other medical community conferences on palliative medicine., • Serve as liaison between the hospice and community physicians., • Make regular contacts with practicing physicians to introduce the hospice program, to educate physicians regarding individuals for whom hospice may be appropriate, and to answer clinical and other concerns of physicians with respect to hospice., • Assist in introducing the VITAS program to long term care providers, managed care providers, hospitals, and others., • Conduct educational seminars, in services, and presentations to physicians, nurses, and other health care audiences whose support for and understanding of the hospice program is integral to assuring that hospice services are made accessible to patients and families., • Education and Research, • Assist in the development of and actively participate in clinical training for all hospice patient care and admissions personnel., • Actively participate in medical and nursing education programs on palliative medicine that may be provided by VITAS to medical and nursing colleges in the community., • Assist in the development of and actively participate in research protocols on both the local and corporate level., • Be a member of and participate in professional organizations related to palliative medicine.QUALIFICATIONS, • Knowledge of the principles and practice of primary medical care, with at least a working knowledge of oncology., • Knowledge of palliative care with particular emphasis on control of symptoms associated with terminal illness., • Ability to work collaboratively with patients' attending physicians to implement the hospice program and effect optimum medical care., • Ability to work collaboratively with hospice employees and volunteers as part of an interdisciplinary team., • Membership in good standing on the medical staff of an accredited hospital in the state in which the hospice is located., • Medical malpractice and liability insurance in amounts and with carriers The ideal candidate will have: • At least 1 year of experience practicing as a Physician Assistant or Nurse Practitioner. Job Description POSITION SUMMARY: The School-Based Health Center (SBHC) Nurse Practitioner manages the SBHC and provides school health nurse services and primary care services to students in the assigned schools. The Nurse Practitioner provides school health services, preventive and acute care, chronic care management, and reproductive health care. The Nurse Practitioner coordinates care delivered and is responsible for billing and reporting requirements, including all appropriate documentation. They report to the School Based Health Center Site Director with support and oversight by the Director of Primary Care and a collaborating physician. The Nurse Practitioner, as the leader of the SBHC, upholds the health center mission by assuring that patients/students receive health care that is competent, caring, cost-effective, and meets with a high degree of satisfaction; and supports the fiscal stability of the organization. ESSENTIAL DUTIES & RESPONSIBILITIES: • Brief Consultations – Assist primary health care providers in recognizing and treating mental health disorders and psychosocial struggles within the primary care setting., • Assess the clinical status of patients referred by primary care providers., • Provide options counseling and referral to prenatal care as needed., • Collaborate/consult with Nurse Practitioners & other clinical staff regarding all patients seen by Behavioral Health Consultant., • Work closely with Providers at the site to identify at risk patients and collaboratively develop plans to prevent further psychological or physical deterioration., • Crisis Intervention – Be immediately available to the Family Practice and Counseling Network staff during working hours in the event of any psychiatric emergency., • Coordinate care of patients with medical/social work/outpatient and outreach staff to treat and manage patients’ chronic emotional and/or health struggles efficiently and effectively., • Refer patients to other levels of care or additional services as needed., • Participate in monthly clinical supervision with an IBH Director., • Participate in monthly clinical/administrative group supervision within Integrated Behavioral Health Department., • Attend and participate in team meetings, case conferences and interdisciplinary work groups/huddles as scheduled within primary care/behavioral health departments., • Ability and cooperation to provide BHC coverage at other clinic sites as needed and requested by Director., • Maintain competencies in mental health as required by all licensing and funding entities; maintain PA license to practice., • Charting - Utilize electronic medical record to document all patient interactions and complete all charting required by contracted funding sources to include assessments, progress notes and billing in a timely fashion., • Productivity – Meet productivity standards as established by the corporation, • Member of Family Practice and Counseling Network’s Multi-disciplinary Team- Attend and participate in meetings and quality improvement activities as required., • Respond to patient or co-worker complaints and work toward a positive resolution of any dispute., • Serve as a member of site committees as requested. Required Education and Experience • Masters in Social Work, Ph.D./Psy.D. in Psychology, or a related field., • Current Pennsylvania licensure as a Licensed Clinical Social Worker, Licensed Clinical Psychologist, or Licensed Professional Counselor., • Excellent working knowledge of behavioral medicine and evidence-based treatments for medical, behavioral, and mental health conditions., • Maintain and uphold Social Work/Psychology ethical and legal guidelines., • Ability to work with a culturally diverse population., • Ability to make quick and accurate clinical assessments of mental and behavioral health conditions via brief patient contact., • High level of comfort in working with primary care medical providers using a consultative style., • Ability to complete clinical documentation with concise, clinically relevant information in a timely fashion., • Demonstrates good time management skills and follow through., • Ability to design effective psycho-education curricula and lead effective psycho-educational group visits., • Ability to work effectively as a team, interfacing with patients, primary care providers, behavioral health staff, as well as with administrative and support staff., • Ability to exercise balanced judgment in evaluating situations and making decisions, and to handle difficult or confrontational situations in a calm, consistent, and equitable manner, • Ability to effectively represent health center’s interests in the community and maintain effective working relationships among co-workers, public, private, and professional groups., • Ability to create a space for staff and those we serve to feel physically and emotionally safe., • Ability to support individuals on their paths to recovery and healing and resist re-traumatization of staff and patients., • Ability to pass required background screenings and credentialing verifications., • Required Immunizations: Hep B Vaccination and TB/PPD Test., • Optional Immunizations: COVID Vaccination and Flu Vaccination., • Current valid CPR Certification required., • Maintain current valid applicable position credentials.
