Our nurses play a critical role in helping patients receive complex therapies safely and confidently in the setting that's right for them. We're looking for a Per Diem Home Infusion Nurse who wants their work to mean something — a skilled Registered Nurse who brings clinical excellence, flexibility, and compassion to patients receiving care in their homes or infusion suites. Why This Role Matters Home infusion requires clinical confidence, independence, and trust. As a Per Diem Home Infusion Nurse, you are often the primary clinical presence for patients navigating complex therapies outside of a traditional hospital environment. In this role, you'll contribute to work that: • Helps patients receive life‑sustaining infusion therapies safely at home or in infusion suites, • Supports continuity of care through skilled assessments, monitoring, and education, • Builds confidence for patients and caregivers through calm, clear communication, • Administer, monitor, and discontinue intravenous therapies in patient homes or infusion suite settings, • Insert and manage IV access including peripheral lines, PICC lines, ports, and midlines, • Monitor patients for adverse reactions and respond appropriately to changes in condition, • Perform venipuncture and collect blood samples as required, • Complete patient assessments, vital signs, and reviews of systems during each visit, • Provide individualized patient and caregiver education on infusion therapy, line care, and symptom monitoring, • Coordinate and communicate visit details, scheduling updates, and clinical documentation with internal care teams and external partners, • Document all patient‑related activities accurately, including medications administered and patient responses, • Assist with planning, organizing, and coordinating patient care delivery across nursing personnel, • Communicate concerns or reportable events promptly to physicians and internal clinical leadership, • Strong clinical judgment and comfort working independently in home‑based settings, • Calm, empathetic communication with patients and caregivers, • Confidence delivering IV therapy outside of acute care environments, • Strong organization skills and attention to detail, • Nursing degree from an accredited college or university, • Current, unrestricted Registered Nurse (RN) license in the state of practice, • Minimum of two (2) years of RN experience, • IV certification and experience with central line care and management, • Valid driver's license and ability to travel to patient homes, • CPR certification, • Experience with patient assessments, documentation, and care coordination, • Basic computer proficiency (Microsoft Office and clinical systems), • Bachelor's degree in nursing, • RN licensure in multiple states, • Prior experience in home infusion, home care, critical care, emergency nursing, or hospital discharge planning Here, you'll find: • Purpose‑driven nursing work with immediate patient impact, • Flexibility through per‑diem scheduling, • A patient‑first, clinician‑led culture, • Strong clinical support and clear care standards While performing the duties of this position, the employee may occasionally be required to stand, walk, or sit; use hands to finger, handle, or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch, or crawl; and talk or hear. The employee may also be required to occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this role include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
