Modifies specific treatment procedures in accordance with PT plan of care and Physical Therapy Assistant (“PTA”) scope of practice. Exercises considerable judgment in accordance with physician's treatment plan and within the scope of departmental policies and procedures. Job DescriptionSocial Worker Long Term Care We are seeking a compassionate, organized Social Worker to join our interdisciplinary care team in a New York State-licensed nursing home. The Social Worker is responsible for addressing residents’ psychosocial needs, supporting their adjustment to long-term care, completing required Minimum Data Set (MDS) responsibilities, coordinating discharge planning, and assisting residents with benefits and entitlement programs. Essential Duties and Responsibilities Complete initial, quarterly, annual, and significant-change psychosocial assessments. Complete assigned MDS sections, assessments, care area assessments, and supporting documentation accurately and within required timeframes. Participate in the MDS process and interdisciplinary care planning, including resident care conferences and development of resident-centered care plans. Address applicable MDS discharge-planning requirements, including Section Q, referrals, documentation, and follow-up, as appropriate.
