JOB DESCRIPTION Job Summary Provides screening, preventive primary care and medical care services to members - primarily in non-clinical settings where members feel most comfortable, including in-home, community and nursing facilities and “pop up” clinics. In this role, they will create registered nurse plans and organize and direct home health and hospice care with an emphasis on community health education/experience. The professional nurse builds from the community's resources to plan and direct services to meet the needs of individuals and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care • Completes an initial assessment of patient and family to determine home care needs.
