Interviews residents to assess functional strengths & weaknesses & develops an individualized support service plan; Works with residents individually & in groups to mobilize inner capacities & resources to improve social functioning. Provides rehabilitative & restorative services to residents as identified in the individual service plan; Documents all interactions with residents and service providers in a timely manner; Maintains communication links with support service agencies who provide residents’ supports & treatments; Assures residents maintain proper activities of daily living & adhere to prescribed medications; Assures residents are in compliance with required rules & regulations. Minimum Qualifications: Bachelor's in related field. Experience working in an apartment treatment program preferred.
