Travel: Coverage Areas: ORANGE COUNTY mostly for east and central Orlando Mileage Reimbursement: Yes Responsibilities: • Conduct comprehensive in-home physical therapy evaluations, assessing mobility, strength, balance, pain, endurance, and home safety, • Develop, implement, and regularly update individualized Plans of Care (POC) based on physician orders, patient goals, and measurable outcomes, • Deliver skilled physical therapy interventions to improve mobility, gait, balance, strength, endurance, and pain management, • Educate patients and caregivers on diagnosis, safety precautions, fall prevention, home exercise programs, and use of medical equipment, • Perform home safety assessments and recommend modifications to enhance safety and support independent functioning, • Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR), • Collaborate with interdisciplinary team members to coordinate care and optimize patient outcomes, • Communicate changes in patient condition or barriers to care promptly to physicians and relevant stakeholders, • Ensure compliance with payer requirements, including documentation standards and eligibility criteria, • HCHB AND OASIS proficient, • Florida PT license, • Current CPR certification., • Valid driver’s license, auto insurance, and reliable transportation.
