Summary: The Director of Care Management Services is accountable for all aspects of SAGE’s Care Management programming throughout New York City, including contract management and oversight and supervision of 12 department staff. The position directly supervises the Assistant Director of Care Management Services, the Assistant Director of Caregiving, the Wellness Counselor, and the Care Management Coordinator, while providing overall strategic oversight and guidance to all of Care Management Services.
