NYCP is seeking a Nourish Intake Associate (English/Spanish) to join our growing Senior Nutrition Programs team. Nourish is a Commodity Supplemental Food Program (CSFP) that aims to increase health in individuals who are 60 years or older and have low incomes by providing nutritious food choices from the U.S. Department of Agriculture. New York Common Pantry is one of the three agencies statewide contracted to run this program, which allows seniors to receive food every month. Food packages include a variety of foods, such as milk, juice, farina, oats, ready-to-eat cereal, rice, pasta, peanut butter, dry beans, canned meat, poultry or fish, and canned fruits and vegetables. Additional components of Nourish include guidance for preparing nutritious meals, dissemination of nutrition and health information by nutritionists, and referral to health care and social service agencies for other needed services. When at full capacity, Nourish will serve up to 80 + partner sites to expand food and nutrition services for low-income seniors living in Manhattan, Queens, Brooklyn, and the Bronx. The Nourish Intake Associate (English/Spanish) will report to the Nourish Intake Manager at 788 Southern Blvd, Bronx, NY 10455. The schedule for this position is Monday through Friday: 9 am - 5 pm; 1-hour lunch. This is a Full-Time position, 35 hours a week, non-exempt. The Nourish Intake Associate (English/Spanish) is required to work on a Saturday three (3) times a year from 9 am to 1 pm. Responsibilities: * Check in and re-certify senior participants at mobile sites. * Entry data into web-based software Link2Feed. The Part-Time Public Health Nutritionist is responsible for planning activities, events, and workshops, and producing outputs that will lead to healthier communities, focusing on serving the SNAP population. The support is needed for Direct Nutrition Education, Farmer’s Market Tours, Farmshare, Food MD, Recipe Club, and many other community nutrition experiences and opportunities. As a part of the SNAP-Ed III Program, LIVE HEALTHY! offers virtual and in-person interactive nutrition education workshops to children, seniors, and adults throughout New York City, aimed at improving the overall quality of life. The Full-Time Food Rescue Driver coordinates and conducts pick-ups of donated food and other items for NYCP's food pantry and hot meal programs. The Full-Time Food Rescue Driver also provides transportation for special events, participates in food drives, and provides program support for the pantry and meal programs to distribute food to individuals and families in need. The Full-Time Food Rescue Driver will report to the Food Sourcing Manager at the 1285 Oak Point Ave, Bronx, NY 10474 location. Additional responsibilities of the Residential Nurse in the IDD Division Consumer Medical Records-Maintain medical record of each individual and ensure all appointments occur as prescribed and on time • Schedule and coordinate all medical appointments in conjunction with Medical Specialist, • Follow up on all Medical Doctor recommendations following every appointment, • Ensure follow up on all medication changes, making sure the order has been sent to the pharmacy, Medication Administration Review has been updated, medication change log updates, bright colored posting of information has been placed and the information transcribed in the AMAP communication log, • Conducting weekly medical meetings( to occur every Wednesday), • Review all consults and ensuring compliance with all recommendations, • Ensure the updating of the Appointment Tracking Log as it is a IDD Division System, • Complete Residential Nurse Progress Note and filing all consults once the progress note has been written, • Ensure all AMAP Certifications are current, • Schedule and conduct all recertification pouring’s, • Ensure all medication supplies of each consumer is present and labeled according to doctors’ orders, • Ensure the consumer’s Medication Administration Review is updates according with all medication changes, neatly organized with formularies and doctor’s order attached, • At least once per week, conduct a full 100% audit of all consumers’ medication and document results on the IDD Division Medication Regimen Review Checklist.
