Job Summary and Qualifications The RN Hematology Case Manager is responsible for promoting patient-centered care by coordinating the plan of care for hematology, transplant, or cellular therapy patients. This role involves collaboration with multidisciplinary teams to assist patients in accessing drug programs, securing insurance approvals for high-cost medications, and coordinating activities that promote quality outcomes, efficient patient throughput, and optimal resource utilization. What you will do in this role: • Coordinate patient care plans for hematology, transplant, and cellular therapy patients., • Collaborate with multidisciplinary teams to facilitate patient access to drug programs and insurance approvals., • Promote quality outcomes, efficient patient throughput, and optimal resource utilization., • Identify and address patient needs during transitions of care., • Act as a liaison between patients, families, physicians, and other healthcare providers., • Identify patients at risk for adverse outcomes during transitions of care., • Perform comprehensive assessments of psychosocial, medical, and discharge needs., • Reassess patient clinical conditions and develop strategies to mitigate readmission risks., • Coordinate care plans and drive discharge planning in collaboration with the healthcare team., • Involve patients and families in identifying needs and setting realistic goals., • Evaluate care progression using evidence-based tools and criteria., • Make appropriate referrals to third-party payers and disease/case management programs., • Facilitate patient throughput with a focus on effective care transitions, quality, and efficiency., • Document professional recommendations, discharge plans, care coordination interventions, and case management activities., • Align patient needs with available resources to ensure safe discharges/transitions., • Act as a liaison between physicians, patients/families, hospital staff, and outside agencies., • Actively seek ways to control costs without compromising patient safety or quality of care., • Direct activities to identify and provide for the needs of under-resourced patients., • Participate in performance improvement activities., • Adhere to established policies, procedures, and standards of care., • Demonstrate knowledge of regulatory requirements, ethics and compliance policies, and quality initiatives., • Serve as an advocate for patient rights, needs, and values. What qualifications you will need: • Florida RN license and/or approved multistate RN license required. Compact license holders have 60 days to obtain the Florida license after relocation., • BLS-AHA Provider issued card required within 30 days of start., • Prefer five years of nursing experience., • Experience in outpatient/inpatient oncology, transplant, and/or cellular therapy is required., • Certification in case management, nursing, or utilization review desired., • Bachelors degree required.
