Job Description APAIT/SSG APAIT, A division of Special Service for Groups, has a mission to positively impact the quality of life for vulnerable communities experiencing behavioral health challenges, housing insecurity, justice-involved individuals, victims of human trafficking, and those at risk for HIV/AIDS and sexual health issues. Learn more about us at Job Summary The Case Manager will be responsible for providing direct program implementation activities as an integral part of the agency’s Behavioral Health and Support Services unit under the direct supervision of the Clinical Program Manager. The Case Manager is responsible for providing intensive, wraparound, field-based case management services. Case Managers serve as members of the multidisciplinary treatment team and support the client's treatment objectives and goal of reducing recidivism and/or increasing their independence in the community. Responsibilities include outreach and recruitment; conducting participant screenings and assessments; facilitating trauma-informed, culturally affirming individual and group level counseling; providing targeted case management; and tracking participant progress. The target population is formerly incarcerated men in a housing site in Highland Park. This facility is all men, and the clients may have the following: severe mental illness, substance abuse, co-morbidity, tri-morbidity, etc. The Case Manager must be comfortable with a setting that focuses on Forensic Treatment. Essential Functions 1. Conduct client intake screenings into behavioral health and psychosocial support program services, and assessment plans., 2. Develop and implement outreach, recruitment, and linkage referrals to community-wide stakeholders., 3. Provide targeted case management services, such as referrals to food assistance, childcare, transportation, housing; care coordination; employment development; skills building., 4. Provide necessary program documentation, including the completion of reports, administration of evaluation tools, and other program related assignments., 5. Facilitate group level counseling sessions., 6. Attend and actively participate in unit, case conference, internal staff in-service, community, and other relevant agency events., 7. Ability to work overtime and perform duties during weekends, holidays, and evenings when required., 8. Regular attendance required., 9. Maintain and uphold Agency mission statement, values, policies, procedures, and principles., 10. Other activities as required to conduct program and agency objectives. Minimum Qualifications - Knowledge, Skills and Abilities Required 1. Bachelor’s degree preferred, OR High school diploma or GED Equivalent and above OR at least 2 years of experience in the HIV/AIDS or behavioral health field., 2. Knowledge of HIV/AIDS, and related issues. • Knowledge of and ability to work with racial/ethnic minority, impoverished and underserved communities and groups (e.g., multi-gender, generation, LGBTQ, substance using). 4. Bilingual in Spanish language preferred., 5. Proficient writing and oral communication skills. • Demonstrated experience and knowledge of PC hardware and software (i.e., Windows, word processing, spreadsheets)., • Ability to coordinate multiple tasks simultaneously in a high-pressure environment., • Maintain and uphold agency mission statement, values, policies, procedures, and principles., • Valid California Driver License and Proof of Car Insurance., • Reliable transportation., • Verification of employment eligibility and background check., • Updated tuberculosis test (annual requirement) – read, reviewed, and dated by a clinician 6 months prior to employment. Preferred Qualifications - Knowledge, Skills, and Abilities • Experience working in a non-profit community-based organization. 2. Experience providing counseling grounded in evidence-based practices such as Seeking Safety or TREM., 3. Bilingual in Spanish language. Supervisory Responsibilities 1. The incumbent in the position does not have formal supervisory responsibilities. Environmental Conditions (Working Conditions) • Ergonomically safe office environment with desktop computer, desk, chair, natural light from windows., • Occasional community outreach that target population frequents., • Some driving and exposure to vehicles in motion. Physical Requirements 1. While performing this job, the incumbent typically spends time sitting, standing, walking, frequent driving, carrying (max.20lbs), lifting (max. 20lbs), listening, and speaking. Mental Requirements Services include individual and group mental health counseling, substance use prevention and treatment, harm reduction and syringe exchange program, HIV/STI testing, prevention, and treatment, transitional and permanent supportive housing, forensic treatment, intensive case management, and research, evaluation, and development.
