An assessment determines the care planning that the care manager must implement as part of the preparation for transition out of the Adult Home. In collaboration with the Nursing Assessor and assigned Care Coordinators, the Social Work Assessor will work with other team members to streamline the assessment and placement into supported housing for identified residents of impacted Adult Homes. Job Description PRIMARY FUNCTION/PURPOSE: The Client Care Coordinator is responsible for the overall delivery and coordination of mental health and related services for homeless individuals. The Client Care Coordinator will enhance existing services to include a thorough assessment of clients’ long-term goals at intake, and collaborate with the case management staff to set achievable goals while establishing linkages within the community to ensure their successful progress towards independent living. if a client did not attend assist with rescheduling & addressing any barriers, • Utilize motivational interviewing if a client does not want to participate or be linked to treatment; attempt to understand your client’s perspective or misconceptions, remove any barriers, and attempt to re-engage, • Provide therapeutic groups (music, art, general support groups, etc.); facilitate group work to address family issues, enhance life skills, etc., • Participate in meetings with case management & housing staff to discuss planning efforts & progress, • Assist clients with completing applications for supportive housing and accompany residents to supportive housing interviews when necessary, • Upon linkage to permanent housing, complete and discuss exit summary with the client, and provide them with referrals and supportive services identified in new community, • Utilize fact sheets, desk guides, and other material resources in delivering mental health services, • Complete all documentation in a timely manner;
