As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you'll manage care plans that support recovery and help patients get back to the life they love. As a Home Health LVN, you will: • Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN., • Implement individualized care plans in collaboration with the patient, family, and healthcare team., • Educate patients and their families on disease management, treatment options, and self-care techniques., • Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes., • Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes.
