Position Summary: Reporting to the Director of Programs, The Assistant Director of Workforce Development exercises significant discretion in overseeing all aspects of employment services for all current and former participants of Youth Action YouthBuild East Harlem and other Youth Action-affiliated programs and services. The primary responsibilities are the development of viable career pathways that link vocational training to educational trajectories, job readiness, career exploration, employer outreach, internships and work experience, employment placement, and reporting. The Assistant Director of Workforce Development ensures that there are viable and relevant opportunities to place YAYB program graduates and alumni in meaningful jobs and careers trajectories along with the requisite varied supports. S/he establishes an effective system to assess and measure participant workforce preparation and readiness, and tracking of student outcomes. The Assistant Director of Workforce Development builds workforce partnerships throughout NYC, but especially in the Upper Manhattan community. They are responsible for achieving all relevant funding contractual goals, performance measures, and reporting requirements. This position also ensures alumni development through close coordination with the Service and Follow-up Coordinator and other program staff as needed. Position Summary: Drives and maintains a vehicle used to deliver meals to homebound elders. Transports a team of delivers to complete all meal deliveries for their assigned routes within an established time frame. The Meals on Wheels driver is also responsible for an individual meal delivery route. Schedule: Between Monday through Friday 20-29 hours per week Hourly Rate: $20.00 Picks-up and delivers meals to homebound seniors. Picks-up and drops-off Delivery staff on the designated route. Observe client and their surrounding area; report concerns as needed. Keeps vehicles clean and in good operating condition, assist with the oversight of maintenance of vehicle. Inspects vehicles daily and reports any operational or structural defects immediately. Reviews vehicle documentation to ensure inspections, registration and insurance information is up-to-date. Maintains accurate daily log of meal deliveries and contributions collected. Assists in scheduling meal deliveries to minimize time and distance between delivery. Keeps daily gasoline and mileage records. Picks-up and returns vehicles to garage or parking place daily. Qualifications High School Diploma is preferred A valid NYS Driver’s License. 2 or more years of driving experience in NYC or high traffic areas. No more than 1 moving violation in 24 months. Able to lift up to 50 pounds. Prior supervisory experience is a plus. Demonstrated sensitivity to an aging population. Bilingual English/Spanish is a plus. Physical Requirements Able to lift up to 50 pounds. Able to work either Thanksgiving of Christmas Day. Summary: The housing Specialist is responsible for assisting and supporting clients in housing resources, benefits, and placement through the entire treatment experience from admission to discharge; Connects and engages the client to housing resources and support services to aid continuity of care and self-sufficiency post-discharge, monitors the course of treatment for each client in a pro-active manner to minimize the barriers that might compromise the client's treatment and post-treatment living environment. This position requires a strong knowledge of appropriate personal and clinical boundaries, a consistently positive attitude, and the ability to deal with difficult, uncertain, and stressful situations in a positive manner. Provides direct housing casework services to clients living with substance abuse histories in our inpatient substance abuse rehabilitation program. Responsibilities: Conducts client housing needs and risk assessment to determine client housing histories and requirements needed during treatment and post-treatment. Collects client data and connects the client to housing entitlements; follow-up with processing and approval for housing readiness Conducts housing services orientation and referrals. Meet with clients on caseload according to regulations and policy to identify client housing needs and progress towards meeting housing goals. Assist client with creating an individualized housing case management plan that will outline the client's goals and housing objectives, as part of the multidisciplinary team. Conducts individual and group services in accordance with a treatment plan including housing options, housing entitlements, benefits, and community resources. Assist with client crisis; communicates with managerial, clinical, and support staff when clients' needs demand additional monitoring or attention when experiencing a crisis, etc. Ability to redirect and de-escalate crisis situations, conduct emergency management, and has an understanding of proper emergency procedures. Document all client care in EHR and records/reports all client/staff incidents as required. Meets with the client according to the care plan to review and discuss discharge plans, continuing care, and community resources needs (continued treatment, entitlements, housing, etc.) Create community housing and entitlement resource banks, make client referrals, and follow up on all referrals. Escort client to appointments as needed. Conduct post-discharge follow-up and document all in the client record and reporting forms. Maintain the charts of clients in accordance with OASAS and agency requirements; Participate in weekly Clinical Team Meetings. Qualifications: Bachelors Degree in social services, counseling, or related area required. Housing case management experience needed. Minimum of two years housing case management experience working with the substance abusing and/or homeless populations preferred. Bilingual Spanish/English preferred. Must have exceptional communication skills both orally and in writing, with individuals and groups regarding confidential and sensitive issues.
