Job DescriptionDescription: The Organization: Founded in 1846 to care for the children and widows of sailors in the Port of New York, Seamen’s Society was one of the first organizations, in the US, to care for abandoned children in family settings, rather than institutions. Today, Seamen’s Society for Children and Families is a comprehensive nonprofit provider of enhanced family foster care services, mental and behavioral health care services, prevention/family stabilization services, a childcare program, domestic violence intervention and counseling services, health home care management services, education financial support, and a youth tutoring program. The agency’s nearly 200 professionals serve over 1,200 children and 800 families in New York City. Job Summary: To provide leadership and support to parent advocates, individual and families. This position plays a pivotal role in providing administrative support to ensure the efficient operation of advocacy services for parents navigating various systems. To provide support to families that promotes a positive parent-child relationship and interaction while providing linkage to parents/caregivers. To support and advocate for the family to receive their entitlements and benefits, housing connects, etc. Duties and Responsibilities: Program Responsibilities · Responsible for opening and closing case records, including sending of all required notices to individuals and families. · Completes all documentation to order gifts cards and Metro cards. · File document and health information case record · Answers phone, greets and announces individuals and families to assigned workers. · Responsible for scheduling, attending, and coordinating trainings for all program staff · compiles and analyze data related to client demographics, service utilization, outcomes, and satisfaction to inform program evaluation and improvement efforts. Documentation and Record Keeping · Complete and document case work activities in tracking logs/excel sheets, case record PROMIS, CONNECTIONS and all other systems on a timely manner. · Conduct monthly chart reviews of at least 10 case records per month. Confidentiality and Privacy: · Maintain strict confidentiality and privacy standards to protect the safety and privacy of individuals and families, adhering to relevant laws and organizational policies regarding the handling of sensitive information. Parent Advocate Case Aide/ Support · Support other parent advocates/case aides on case assignments if needed. · Advocate and assist families with connecting to community-based resources/ organizations and making referrals to facilitate the services needed to the families. · Participate in activities including initial intakes, contacts, and service plan meetings, etc., · Obtain collateral documents quarterly and/or annually as provided, from medical, mental health treatment, substance use and academic programs and other programs. · Works with parents and provides education on improving parenting skills and home management skills. · Schedule and participate in monthly unit, Interdisciplinary, and/or Clinical Team Meetings. · Ability to conduct interviews with families to obtain and provide information needed to fulfill case aide responsibilities. Skills, Knowledge, Abilities: · Ability to use communication skills that successfully convey and obtain accurate and complete information in a respectful caring manner. · Demonstrates ability to organize and coordinate work within schedule constraints and manage emergent requirements in a timely manner. · Ability to communicate effectively with supervisors, co-workers, and other community partners · Knowledge and ability to use critical thinking and analytical skills used to formulate with the family person achievable plans for implementation · Ability to perform the essential functions of the job with or without reasonable accommodation. · Collaboration and facilitation skills that to collaborate with the individual, families, and services providers. Qualifications: · HS Diploma or GED required; child welfare experience preferred. · Computer skills including but not limited to Microsoft Word, PowerPoint, SharePoint, Teams, and ability to learn other systems. · Due to the nature of the work environment work hours will be based on needs of program and may vary. · Performs other duties/responsibilities as assigned within scope of this position.
