Job Description Purpose of the Role Using a status-neutral approach, the HIV Youth Services Specialist engages LGBTQIA+ youth, ages 13-24, through outreach, testing/screening, linkage, education, and supportive services. They conduct HIV/STI testing and screening for all youth, access to PEP/PrEP/Doxy-PEP and contraception, and linkage to care services through navigation support, facilitate evidence-based interventions, and connect youth to sexual health care and other essential resources – such as mental health care, housing support, Pantry services, medical appointment support, assistance enrolling in insurance, and more. Using a trauma-informed, sex-positive approach, the Specialist promotes risk reduction, prevention, and youth empowerment. Essential Functions: % Time Role Description 60% Contractual Obligations 1. Perform screening and assessment of youth within HMI and within the community using the GOALS framework to ensure they are offered routine HIV/STI testing and screening services, conduct follow-up navigation and case management support, provide appropriate referrals and linkage to supportive services. a. Conduct external outreach during major public health observance dates (National Youth HIV AIDS Awareness Day, National HIV Testing Day, Zero HIV Stigma Day, and World AIDS Day) and pride events (all month of June); conduct HIV/STI screening during these activations b. Outreach and engage youth in need assessment and HIV testing in HMI milieu and at external HMI outreach events c. Conduct at least 30 monthly HIV tests on-site and within community setting. d. Conduct one-to-one interviews for case plans and referrals to care, preventative services, or other supportive services for youth and provide scheduling and appointment support. e. Link clients to approved community health partners for navigation referrals and linkage to care. f. Assist with youth escort to care for HIV-specific medical services. g. Provide case management services for YPLWH to help with treatment adherence and retention in care. h. Connect youth to HMI mental health services, when necessary. i. Work closely with other HMI programs to ensure appropriate navigation of services for youth. j. Maintain individual client files and adequate paperwork, as well as electronic records 2. Assist with improved access to biomedical prevention services for priority populations, including: a. PrEP/PEP/Doxy-PEP education. b. Linkage navigation to clinical sites providing PrEP/PEP/Doxy-PEP and other services. c. Follow-up activities including access to mental health services, 3. Facilitate participation in on-site health /medical screenings, through use of community partners [HEAT, Callen-Lorde Mobile Medical Unit, & APICHA]. a. Recruit and engage young people throughout HMI to access medical screenings offered on-site. b. Create referral systems for youth medical care engagement and follow-up. c. Maintain client level data, including follow-up notes and referral logs 15% Programmatic Direct Service 1. Coordinate and facilitate at least two 8-week evidence-informed interventions annually and monthly sexual health activations for LGBTQIA+ youth, ages 13-24, living in NYC. a. Recruit, educate, and train youth in risk-reduction and prevention strategies regardless of their HIV/STI status b. Initiate, coordinate and develop promotion material related to educational intervention, along with youth c. Conduct at least 10 workshops related to sexual health/HIV/STI prevention programming to HMI community at large 2. Work closely with other HMI departments and staff to support ongoing programs and services. a. Outreach and engage youth during Program Common Hours (3:30pm – 7:30pm) b. Prepare for and facilitate weekly group-based activities for general programming, as needed c. Perform general staff duties, such as Café duty, general event set-up/breakdown, etc d. Participate and assist with general HMI agency-wide events 25% Administrative Duties Track youth member enrollment/activities and contractual requirements, and prepare reports and paperwork requested as needed by agency or funders. a. Maintain client level data, including follow-up notes and referral logs. b. Maintain group level documentation, including sign-in sheets and session summaries. c. Document, code and enter data for program analysis. d. Submit all necessary documentation to Data Coordinator(s) As Needed. Other duties as assigned. Knowledge, Skills and Abilities Required: • Bachelor's degree in health education, public health, or related human services field., • Experience conducting HIV screening and test, along with providing results., • Experience completing and filing legal documents accurately and efficiently., • At least two years' experience with diverse population of adolescents in an urban setting, including knowledge of lesbian, gay, bisexual transgender and questioning youth from various cultures and backgrounds., • Experience facilitating group discussions and managing a variety of small and large group dynamics., • Experience planning and implementing health-specific programs., • Excellent verbal and written communication skills, including conflict mediation and public speaking., • Knowledge of harm reduction practices, trauma-informed care, and positive youth development models., • Ability to exercise independent judgment when appropriate; strong decision-making skills., • Computer Skills: Strong with proficiency in Microsoft Word, Excel, PowerPoint and Outlook Additional Desirable Qualifications: • Extensive knowledge of one or all of the following health and wellness related issues: Sexually Transmitted Infections, Sexual and Reproductive Health, Pregnancy Prevention, Substance Use, Nutrition, and Violence prevention., • Bi-lingual (Spanish-English) skills.
