Program/Department Description Community Access’s OMH licensed Intensive Crisis Residence (ICR) is inspired by the Soteria model and is designed as an alternative to psychiatric hospitalization for people going through an acute psychiatric crisis and who would benefit from a respite period in a stabilizing, home-like environment with 24-hour peer support and treatment services. The Soteria ICR will work with guests for up to 28 days to help them develop awareness and skills that will allow them to return to the community with an increased ability to manage their mental health concerns. The ICR will follow a Soteria model which emphasizes shared running of a home-like environment, positive expectations of recovery, validation of subjective experiences, the concept of “being with” a person, providing 1:1 support, 24 hours a day, and self-determination. Services are completely voluntary, and guests are encouraged to maintain their daily responsibilities and activities in the community during their stay including work, school, family care, recreation, and wellness appointments. The program works with guests throughout their stay on discharge planning focused on safety and wellness, and provides post-discharge follow up health resource navigation support. This knowledge is continuously expanding and changing in response to research, development of new treatment techniques and psychoactive medications, and greater understanding of psychiatric and developmental disorders, treatments, and health care management. Psychiatrists must maintain high levels of current knowledge about new developments in psychiatry. Further, they must apply this knowledge while managing people who may have limited or no understanding of psychiatric and developmental disorders or are not responsive to treatment for reasons other than their condition and the appropriateness of the treatment. Psychiatrists are regarded as experts in the field of psychiatry by persons in their care, an individual’s caregivers, other direct care staff, treatment team members, and other medical staff. They provide expert psychiatric advice about patient conditions and treatments, while considering a wide variety of factors that impact the general well-being of those individuals under their care. COMMUNICATION: Good communication and listening skills are critical for the performance of duties and effectiveness of treatment prescribed and conducted by Psychiatrists. Psychiatrists have frequent oral communications with patients to conduct psychiatric evaluations and individual and group therapy; additionally, they must be able to observe and comprehend non-verbal communications to understand and evaluate patient conditions and responses to treatment. Psychiatrists have frequent oral communication with caregivers, patients and their families, treatment team members, and other medical, nursing, and support staff to elicit and clarify information, answer questions, explain conditions and treatment methods, coordinate treatment, give orders for patient care, and provide advice. They must explain complex information so that it is easily understood and comprehended by patients and caregivers, who generally have limited or no understanding of medical science and the field of psychiatry. Psychiatrists have frequent written communications to prepare psychiatric evaluations, record information about patients and their response to treatment, prescribe medications, and maintain patient records. In addition, written communications include reports about the treatment programs of the program, presentations of papers about research and treatment, explanations of new developments in psychiatric care and treatment, and dissemination of procedural, policy, and other information affecting unit and facility operations. Psychiatrists are legally responsible for the accuracy of psychiatric information that they convey. SUPERVISION EXERCISED: Psychiatrists supervise staff by prescribing treatments, providing orders to implement and revise treatment programs, supervising the administration of treatment, answering questions and advising staff about patient care and treatment. MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS: ADULT PROGRAMS Board Eligible: • Unrestricted license to practice medicine in New York State and completion of a post-graduate residency training program in General Psychiatry approved by the American Board of Psychiatry and Neurology for entrance into their examination, and, • Eligibility for full and unconditional participation in the Medicaid and Medicare programs. Board Eligible • License to practice medicine in New York State and completion of a post-graduate fellowship training program in Child & Adolescent Psychiatry approved by the American Board of Psychiatry and Neurology for entrance into their examination, and, • Eligibility for full and unconditional participation in the Medicaid and Medicare programs., • License to practice medicine in New York State and, • Certification in General Psychiatry or Child & Adolescent Psychiatry by the American Board of Psychiatry and Neurology or by a foreign board clearly equivalent and, • Eligibility for full and unconditional participation in the Medicaid and Medicare programs., • Possession of current Controlled Substances Registration certificate from the US Drug Enforcement Administration. FOR ALL POSITIONS: Continued employment is based on the ability to maintain professional licensure, registration and participation in the Medicaid and Medicare programs. PREFERRED EDUCATION AND/OR EXPERIENCE: Experience providing services to target age groups, working as part of a multi-disciplinary treatment team in an outpatient setting.
