Overview Provides clinical leadership to promote increased compliance with a range of quality and cost measures and standards of care. Manages service delivery of inter-professional and para-professional team members working on an individual case or population of cases. Acts as a key resource in providing clinical and operational guidance and support to assigned teams and other staff to achieve and enhance team outcomes. Evaluates patient responses to therapy / interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes. * Identifies need for new/revised clinical protocols. Collaborates with physicians and others within the practice to develop protocols and provides training as appropriate. * Manages and provides comprehensive advanced nursing care including physical examination, comprehensive history, screening for physical and/or psychological conditions, emergent interventions, pharmacological and non-pharmacological interventions, ordering treatments and DME, preventative health maintenance activities, care management, referrals, discharge planning, counseling and patient education. Establishes a treatment plan based on clinical findings and. Determines when further evaluation by collaborating physician, specialist or emergency care is warranted. * Collaborates with patients, families, primary care physicians and other team members to provide assessment and care planning. Assesses, plans, and provides intensive and continuous care management across client settings. * Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements. Updates plan at specified intervals, and as needed based on changes in client/member condition or circumstances * Performs and maintains effective care management for assigned caseload of clients/members. BJI is seeking a Court Liaison, Supervised Release Program. Reporting to the Supervising Court Liaison, the Court Liaison will coordinate with court stakeholders to ensure the successful release of program participants to the Brooklyn SRP. The Court Liaison will review all available court-related information about each participant released under supervision to complete the necessary paperwork. Once participants are enrolled in programming, the Court Liaison will provide written and verbal updates to the court, including defense attorneys, assistant district attorneys, and judges, on a regular basis. Job Description POSITION SUMMARY The Client Coordinator is responsible for ensuring that overall operations and services are delivered according to state and federal regulations, agency policies and procedures, and the agency’s mission. The Client Coordinator provides leadership, direction, and training to support the overall development of a specific program. Utilizing the Interdisciplinary Team approach, the Client Coordinator develops implementation strategies that outline services and supports for individuals as they work toward the outcomes detailed in their Personal Plan. The Client Coordinator oversees the treatment process, ensuring implementation of team recommendations by all members of the Interdisciplinary Team. They are responsible for ensuring service provision by all disciplines represented on the treatment team, and meet routinely with other clinicians for coordination and integration of services. The Client Coordinator coordinates with Program Coordinator(s) and staff to provide individual and day program services, and to maintain a therapeutic milieu conducive to participants’ health and well-being. They coordinate and oversee the development and implementation of each Life Plan in accordance with regulations. For new individuals, the Client Coordinator works with the Intake Department to complete required documentation for compliant intake procedures. The Client Coordinator ensures timely completion and submission of required IDT documentation to the Day Program Director or designee and the team. They hold primary responsibility for developing program goals and methodologies and supporting DSPs to correctly implement and document such activities. The Client Coordinator ensures integrated services by fostering team awareness of service integration, coordinating service delivery with day program services, monitoring service provision by agency clinical staff, consultants, and community vendors, and ensuring services align with IDT treatment recommendations. They ensure a therapeutic environment through regular staff and individual observations, initiating corrective actions when necessary to maintain adequate coverage levels and ensure an atmosphere conducive to development and well-being. They document progress and update Plans of Care / ISPs according to agency procedures. The Client Coordinator reports regularly to the Supervisor regarding program compliance, noncompliance, and remedial actions. They identify needs and request support as necessary. The Bilingual Facilitator will teach primarily in the Tactics and Choices class, which facilitates classes for those arrested for Intimate Partner Violence (IPV) charges and who are participating in the Supervised Release Program. Participants are either mandated or encouraged to voluntarily attend this class as part of their supervised release program. The class offers participants an overview of basic principles of power and control dynamics and uses an interactive format to introduce strategies for making healthy choices in intimate relationships. The Bilingual Facilitator will provide this class as part of a larger IPV supervised release track, with the goal of increasing compliance with court mandates and return to court. The Bilingual Facilitator will also train in other IPV and domestic violence groups and classes as the team seeks to expand programming. The Bilingual Facilitator will primarily teach online but will is also required to teach classes in person throughout New York City's five boroughs as the program expands. The class will be offered in several separate groups: Spanish-speaking, male, female, and LGBTQ+ defendants utilizing different curricula. Classes are offered in other languages with an interpreter on an as-needed basis. Job Summary: This position requires an individual who is especially detailed-oriented and has strong written, communication skills, and counseling skills, particularly in trauma-informed and client-focused practices. The selected candidate should be comfortable identifying and pursuing creative problem solving that is necessary for survivors of domestic violence. Job duties include but are not limited to conducting intakes, administering assessments, providing or seeing to the provision of necessary social services for survivors of domestic violence, providing housing assistance to all clients, including clients transitioning from shelter and collaborate with the Safe Passage team to ensure that all survivors receive quality services. Duties and Responsibilities: · Assist individual clients with obtaining a security alert system · Enter all information in the NYC HRA IRM system · Provide direct case management and support to individuals on caseload. · Assist individuals with flex funding applications. · Attend and participate in internal and external meetings. · Refer clients to safe passage and Children Adolescent Servcies (CAS) programs. · Provide housing assistance to all clients, including clients transitioning from shelter. · Assist survivors with housing vouchers, Section 8, City FEPS/FHEPS, NYCHA. · Assist in finding permanent housing and safety transfers. · Dispute lease agreements with landlords and building management. · Establish linkages with domestic violence shelters so that families discharged from shelters are supported in their new community. · Directly provide (or give referrals for) homeless prevention services. · Address 24 hours helpline crisis calls and walk-ins, documenting counseling and other services provided. · Conduct comprehensive assessments with all clients and refer children/individuals/families to supportive and concrete services such as housing assistance, financial assistance, education, substance use, mental health, medical, immigration, legal, etc. · Conduct assessments within 30 days of intake and review assessments with clients every three months after intake until goals are met, and the case is closed. · Provide supportive counseling, case management, and crisis intervention. Work collaboratively with survivors to define case goals, objectives, and action plans. · Meet program expectations for client contacts. Will contact client within 24 hours due to the critical nature of the work. · Advocate on behalf of clients to overcome systemic barriers related to court, immigration, housing, public benefits, etc. · Facilitate/co-facilitate groups for clients. · Develop content, schedule, and participate in monthly outreach events and document attendance of participants. · Collaborate with internal and external partners to advocate for survivors in court matters, housing matters, public assistance, immigration, etc. · Complete all data collecting and reporting promptly.
