Job Description SERV, a well-established leader in behavioral healthcare in New Jersey, supports people with mental illness and/or intellectual/developmental disabilities as they achieve greater independence and life satisfaction through a wide range of services. Reporting to the Chief Operating Officer, the Advanced Practice Nurse is responsible for assessment, diagnosis, and treatment of illnesses that occur consumers enrolled into the substance use program. The Advanced Practice Nurse will enter into a Collaborative Agreement with a psychiatrist and adhere to all regulatory requirements. The Advanced Practice Nurse will document in the clinical record in a timely manner and adhere to all consumer privacy and confidentiality practices. With a team of over 900 staff and an annual budget of $130 million, the Center carries out its mission through three core strategies: • Operating Programs that pilot new ideas and address local challenges;, • Conducting original research to evaluate what works—and what doesn’t; and THE OPPORTUNITY The Center for Justice Innovation is seeking a Senior Social Worker to join a team of staff—including social workers and case managers—who work with defendants participating in the Manhattan Felony Alternative to Incarceration (ATI) Court. The ATI Court, which is led by Administrative Judge Ellen Biben, seeks to forge a new response to individuals charged with felonies by linking them to community-based social services. The Court is the first of its kind, providing an innovative model for other jurisdictions seeking to develop and implement meaningful post-conviction sentencing options for individuals facing felony charges. Reporting to the Clinical Director, the Senior Social Worker will be part of a team that provides neutral, third-party assessments and case management services to the ATI Court. The Senior Social Worker will conduct independent clinical assessments, provide referrals to community-based services, offer comprehensive clinical case management, and monitor participant compliance. The Senior Social Worker will be primarily responsible for supporting a caseload of high-risk need program participants and supervising a group of case managers and/or social workers. Additionally, in coordination with the Clinical Director, the Senior Social Worker will help coordinate compliance updates on ATI Court calendar days, help identify gaps in services and forge new partnerships with community-based providers to fill those needs, serve as the team’s training liaison to identify internal training needs and coordinate trainings for staff, and coordinate with the Office Manager regarding participant supplies and other needs. An essential and dynamic feature of the Senior Social Worker’s role will be in close collaboration with key stakeholders. The Senior Social Worker will work regularly with the judge and court staff from New York County Supreme Court who oversee the Felony ATI Court, as well as with other stakeholders, including the Manhattan District Attorney’s Office and New York City defense organizations. Court-based social work is an exciting, unique, and demanding area of practice, and the Senior Social Worker role involves navigating and constructively engaging with differing and, at times, competing stakeholder perspectives. Administrative and Advocacy Role • Prepare and maintain timely and accurate FASP documentation, monthly statistical reports, and progress notes for each session in adherence with agency policy utilizing PROMIS, Connections, and other required databases., • Participate in all SBC model training, supervision, consultation activities, as well as program and agency staff meetings and trainings., • Ensure certification in SBC model within the expected timeframe by utilizing and documenting key practice skills and submitting required documentation and information as required by model developer., • Collaborate closely with ACS personnel, school staff, medical and mental health clinics, and other stakeholders serving family members in order to make sound and effective decisions., • Attend and participate in Family Court hearings for families involved with ACS Family Services Unit (FSU), providing Court reports as needed to inform as to families’ progress or lack of progress in meeting goals., • Provide consultation to and collaborate with the program’s Family Workers as needed to ensure high quality case management services for families., • Represent participant needs, concerns, and perspectives at all required ACS and GSS meetings and trainings. In this role, you will provide care coordination and health promotion services to individuals throughout Long Island. Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care.
