Job DescriptionDescription: POSITION SUMMARY: This position supports the work of the Y, a leading nonprofit committed to strengthening community through youth development, healthy living and social responsibility. Provides direct leadership, instruction and motivation for students by teaching youth classes in the arts and enrichment to various age groups. Creates positive, nurturing relationships with children, while building cooperative relationships with parents/caregivers. Requirements: ESSENTIAL FUNCTIONS: • Program Assistants will assist with designing and delivering lesson plans in the designated area tailored to the skills and interests of each age group., • Assist in instruction of a group in accordance with YMCA guidelines., • Assist in planning, setup and delivering a variety of activities within interest area that are age specific and that keep children’s engagement and interest high., • Builds effective, authentic relationships with students and parents; helps them connect with each other and the YMCA. Security Level: Shared Staff - FLIPA Duties/Responsibilities: • Provide a vital link between local communities and healthcare provider by helping individuals access resources and navigate systems., • Proactively outreach and engage identified individuals in need of services, follow up or social care screening by connecting via phone calls, home visits and/or in-person visits to other settings where patients can be found., • Support deployment of NYS Social Care Network screening and referral process, • Engage directly with Medicaid individuals to administer the Health-Related Social Needs Screening Tool to identify needed areas of support., • Facilitate referrals to appropriate community resources and healthcare providers., • Collaborate with the Care Team to ensure timely follow-up and service linkage., • Use designated online referral systems and databases to track and manage client referrals., • Provide care management related to social care services, • Accurately document in electronic systems and maintain detailed and organized records in compliance with organizational policies and standards., • Work closely with the Care Team, including care coordinators and other healthcare professionals, to align to a whole person care approach., • Participate in regular team meetings and contribute insights on client progress., • Attend regular supervision, staff meetings, trainings and other meetings, as requested., • Other duties as assigned Education & Experience: • Minimum of High School Diploma or GED., • Associate's degree in human services, Social Work or other related degree preferred., • Equivalent experience in lieu of education may be considered., • Minimum of 1-3 years' human services experience. The New Rochelle Community Justice Center is seeking a Program Associate for its DREAM (Developmental Resources for Emerging Adult Members) and YJEP (Youth Justice and Equity Program) programs. Reporting to the Program Coordinator, the Program Associate will lead the presentation of workshop curriculum objectives and activities, while also coaching and supporting program participants towards individual goals. Responsibilities include but are not limited to: • Assist with the creation and development of program curricula and workshops that promote essential life skills and wellness for emerging and young adults;, • Lead multi-week training cohorts that include discussions on personal development, future outlook, and civic engagement;, • Build and maintain a caseload of participants, and develop and implement a personalized individual action plan with each participant;, • Provide participant coaching toward stated goals and identify areas of opportunity for referrals or further development;, • Contribute to data collection and management using Salesforce and other tools;, • Maintain electronic program materials;, • Represent NRCJC and its interests at meetings and events as assigned; and, • Additional tasks as necessary.Qualifications: Bachelor's degree and preferably 1-2 years of related experience or a high school diploma or equivalent and at least 5-6 years of relevant experience facilitating workshops or holding group discussions or restorative circles. Candidate must be proficient in Microsoft 365 and comfortable using data management tools.
