Job Description Position Overview: As a Mobile Wound Care Nurse Practitioner/ Physician Assistant, you will be responsible for providing expert wound care management in patients' homes and/or care facilities. You will assess, diagnose, treat, and educate patients with chronic and complex wounds, including but not limited to diabetic ulcers, pressure ulcers, surgical wounds, and other skin-related conditions. You will work collaboratively with physicians, nurses, and administrative leadership to ensure the best possible care for our patients. Job Description *Registered Nurse II NICU *24 Hours per week *Night shift *Rotate Weekends/Holidays SLH NICU is a collaborative environment with a robust multidisciplinary team including in-house, 24/7 coverage by a Neonatal Nurse Practitioner and Neonatologist. Our Registered Nurses arehighly trained and specialized nurses who work with newborn infants who are critically ill and/or premature. These nurses provide care for infants in the neonatal intensive care unit and work closely with a team of healthcare professionals to ensure the best outcomes for the infants in their care. The role of a NICU RN is both challenging and rewarding, as they are responsible for the well-being of some of the most vulnerable patients in a healthcare setting. The NICU RN is responsible for monitoring vitals, managing central venous and arterial lines, chest tubes, enteral and oral feedings and performing other patient care needs. Our NICU runs four types of ventilators; HFOV, HFJV, Servo and conventional that the RN manages jointly with the respiratory therapist.We strive for a family-centered model of care and value family input and participation. We have three zones (Red, Yellow, and Green) and nurses are scheduled in 4-hour blocks, typically rotating to zones once or twice in their shift. What You Will Do • Provide Direct Patient Care – Deliver on-site and virtual supervision while administering infusions and injections with precision., • Conduct Patient Assessments – Perform evaluations, document SOAP notes, and monitor treatment responses., • Educate and Support Patients – Guide patients and families through treatment expectations, potential side effects, and disease management., • Ensure Compliance and Quality – Maintain accurate records in the Electronic Health Record (EHR) system, review nursing documentation, and uphold HIPAA and OSHA standards., • Optimize Clinical Workflows – Assist with chart preparation and treatment planning to streamline patient visits., • Collaborate with Healthcare Teams – Work closely with referring providers and cross-functional teams to enhance care coordination., • Support Operations and Training – Assist in inventory management, prior authorizations, and regulatory adherence., • Flexibility and Adaptability – Work at multiple facilities within a designated market as needed., • Active and Unrestricted Licenses – Nurse Practitioner (NP) license and Registered Nurse (RN) license (if required by the state)., • Board Certification – AANP or ANCC certification required., • Life Support Certifications – BLS certification required; PALS certification preferred., • Clinical Experience – Background in an ambulatory infusion center, outpatient specialty clinic, or similar setting is a plus., • Ability to Treat a Diverse Patient Population – Must be able to care for both adults and pediatrics—Family Nurse Practitioner (FNP) preferred., • Strong IV and Infusion Therapy Skills – Experience with specialty biologics, medication management, and emergency response protocols., • Proficiency in Clinical Documentation – Experience with EHR systems and a strong understanding of HIPAA and regulatory compliance., • Exceptional Communication & Collaboration – Ability to work effectively with patients, families, and multidisciplinary teams. Responsibilities About the Role During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: • Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols, • Administer and order point of care and lab-based testing as needed, • Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate, • Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary, • Flexibility – Visits are scheduled based on YOUR availability. We conduct assessments Sunday – Saturday from 8 am – 7 pm. You can work during the day, evening, and/or weekend based on your schedule., • Competitive Pay – Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability. Those completing a monthly average of 20 – 100 visits earn $1,300 – $6,500 per month.
