Job Description Essential Duties & Responsibilities * Answers questions and provides information to patients on compound ingredients, dosage, storage medication(s), use of medication(s), pertinent interaction(s) and other information relating to the compounded medication(s) * Mixes pharmaceutical preparations and appropriately labels containers * Establishes and maintains compounding records and enters prescription data accurately * Assists the clinic team in providing patient care * Assists in maintaining inventory of the IV area, including, but not limited to: labels, medication, compounding ingredients, vials, etc. * Cleans and helps maintain equipment and work areas according to SOPs * Maintains proper storage and security conditions of vitamins, ingredients and finished compounds * Follows established SOPs, assists with formulation development of new SOPs and data entry, and competence in mathematical calculations * Effectively communicates with prescribers and other healthcare professionals, conveying appropriate information to designated staff for proper compounding of IV vitamins and IM Quickshots * Attends professional meetings and interacts professionally with patients, staff and external associates as needed * Delegates tasks to other staff members according to individual strengths/weaknesses. * Oversees SOP implementation, training verification, formulation of medications, CQI, quality of ingredients, staffing requirements, patient-specific needs, and any other aspect of work in the IV area * Evaluate, plan, implement and document nursing care for clients * Prepare IV & IM vitamins for administration with appropriate technique * Inserting, monitoring and removing IV - ensuring needles and tubing stay in place during treatment, and procedure is done as easily and comfortably as possible. Oversight of the client during the infusion process * Reviewing treatment plans and ensuring the clients are up to date with labs and f/u appointments * Clean and maintain infusion room & sterile hood * Various Office and Customer Service duties to include but not limited to scheduling appointments, verifying the correct forms are filled out and complete, reviewing health history forms, and providing excellent customer service * Performs venipuncture. Possible accesses implanted ports or PICC lines * Orders and maintains adequate IV supplies * Triage's patient phone calls as needed and documents relevant information in the client’s chart * Follows Daily, Weekly, Monthly Requirements for Lab and staff * Additional tasks as needed Qualifications * Continuous speaking, hearing and visual effort. * Frequently required to walk, stand and sit. Job Description Job Summary The dedicated and collaborative Shift Supervisor is responsible for maintaining a safe, supportive, and therapeutic environment which fosters growth and facilitates the development of mutual aid/self-help skills with the people we support. The ideal candidate will supervise staff and provide the appropriate supports tailored to the needs and desires of each person being supported to enable them to live as independently as possible. The Shift Supervisor is expected to recognize when an individual is in a crisis and be able to utilize the appropriate psychiatric and medical back up services. We have open Shift Supervisor positions at our group homes in St. Albans, Astoria, and Jamaica Estates. Maintains written logs for recording daily activities., • Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment., • Supervises any other activities as may be necessary to maintain quality of the program., • Dispenses prescribed medications to the people we support after successful completion of the medication administration course, and in line with Agency protocol. Job Description Summary This position is primarily responsible for oversight and management of focused outreach, administering regulations, policies, procedures and practices; planning, organizing, developing, implementing and coordinating program activities for the Center of Innovation (LEAP) Scope of Work. The Program Manager reports to the Director of Outpatient Services and supervises the LEAP program team. Essential duties and Responsibilities: Core duties and responsibilities include the following. Other duties may be assigned. • Coordinates and integrates service delivery within the program., • Ensures program compliance with OASAS regulations and program policies directly related to program operations and record management., • Coordinates with Quality Assurance/Data Specialist to ensure accurate data collection and reporting, • Provides community education, coordination of community-based initiatives, and implementing initiatives to promote health and safety of individuals affected by drug and alcohol use., • Develop program services that meet OASAS standards for medically assisted treatment provided through telehealth, in community settings, and existing substance use treatment providers., • Direct planning, coordination, and follow-up of outreach to the community., • Coordinate with the nurse practitioner and medical doctor to ensure MATS treatment compliance., • Develop and manage peer support services., • Develop program protocols to improve service delivery and participant outcomes., • Works collaboratively with the Director of Outpatient Services to ensure effective program operations., • Ensure staff maintains accurate and confidential clinical records that comply with OASAS standards and internal policies., • Manage participant data, statistics and tracking for quality improvement and monthly reporting., • Responsible for the implementation of interventions to address the ongoing expansion of overdose prevention, • Provides crisis interventions and consultation as needed, • Manages and responds to crisis in the field, • Monitors participants’ services, assuring that staff are responsive to participant needs and take appropriate action to make adaptations as necessary., • Evaluates staff performance; identifies documents and develops corrective action plans for personnel performance problems. This includes directing staff, providing consumers with proper care, ensuring the safety of the consumers, scheduling necessary repairs and the general upkeep of the facility. Administers designated program in accordance with Agency policies and regulations and requirements of applicable regulatory agencies. Is responsible for all aspects of program including personnel, budgeting, services to individuals and physical plant management. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Director of Residential Program Services – Residential Services will include, but are not limited to the following: • Works closely with Interdisciplinary Clinical team to coordinate residents’ treatment plans., • Ensures that all measures for safety and well being are maintained at highest level., • Provides overall supervision to residence staff including Registered Nurse/Licensed Practical Nurse, Residence Manager, Senior Residence Program Specialist, Case Manager, Residence Program Specialist, Recreation Specialist utilizing flexible time scheduling to make on-site consultations and supervision of each shift., • Conducts weekly/biweekly staff meetings with all direct care staff., • Interviews, evaluates, hires, orients, trains, disciplines and when necessary, terminates subordinate staff., • Maintains and posts scheduled work assignments and adjusts such assignments based on availability of staff, individual abilities of staff and particular requirements of the people we support., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action in absence of a nurse., • Ensures actions are conducted in a manner that prevents abuse., • Administers appropriate first aid based on visible symptoms and resident history in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • Maintains records on subordinate staff such as level of performance, specific deficiencies, counseling sessions training records, etc., • Conducts weekly individual supervisory meetings with subordinates in order to discuss program, personnel or training needs and assesses their performance., • Responds to program and work-related questions, problems, complaints and grievances from staff., • Develops residence facility rules and procedures., • Ensures residence facility is kept clean, neat, healthy and safe., • Assures that all staff are coordinating their efforts in order to complete such routine household tasks as laundry, menu preparation, shopping, cooking, etc. with goal of increasing independence of residents in performing these functions., • Directly responsible for all medical issues of residents., • Works with appropriate clinical or program staff and community agency staff to identify and maintain placement in day programs, workshops, school programs, etc. which are in community and are required in residents’ service plan., • Acts on behalf of residents at schools or school boards, health clinics, local businesses, employers, transportations, providers, social groups, neighbors, etc. in order to ensure fair and appropriate consideration. SUMMARY Under general supervision, is responsible for devoting their attention to the care and development of the people we support in one of the Agency’s residential facilities. Interacts in a manner which fosters dignity and learning in a way that helps lead to greater independence. Helps create a homelike atmosphere for the people we support in an agency facility. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following: • Provides care and assists with personal development and independence of the people we support., • Ensures that all measures for safety and well-being are maintained at highest level., • Provides training for the people we support in activities of daily living and in development of self-help and social skills., • Maintains a personalized, homelike atmosphere in the residential facility., • Attends weekly staff meetings as well as any other assigned meetings and in-service trainings., • Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action., • Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures., • Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines., • Provides transportation for the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc., • Assists with meal planning and preparation as well as with purchasing of food and supplies., • Demonstrates an understanding of and implements behavioral programs., • Participates fully as a member of interdisciplinary plan., • Maintains an ongoing informal exchange of information. Maintains written logs for recording daily activities., • Performs housekeeping, cooking, laundry, light maintenance and light preventative maintenance duties as needed to maintain a safe and homelike environment., • Supervises any other activities as may be necessary to maintain quality of the program., • Dispenses prescribed medications to the people we support after successful completion of the medication administration course, and in line with Agency protocol. AMAP’s are supervised by residential nurse in all areas related to medication administration., • Completes all activities related to skill buildings as outlined in individual program plans, • Provides support around personal hygiene, i.e. toileting, bathing and changing as needed., • Handles all personal documents of the people we support in a secure and confidential manner., • Performs related duties as requested. Job Description The American Lung Association has an excellent opportunity for a Manager, Health Promotions. Working as a member of the Health Promotion & Education department, you will join an outstanding group of professionals dedicated to our mission: to save lives by improving lung health and preventing lung disease through research, education, and advocacy. Manage and implement Addressing the Burden of Childhood Asthma in New York State grant including evidence-based strategies to expand the quality and availability of guidelines-based asthma control services. Efforts aimed at improving asthma-related health outcomes and quality of life among children with asthma and their families/caregivers across New York’s highest asthma burden areas will be driven by the strategies outlined in the Center for Disease Control (CDC) National Asthma Control Program’s EXHALE Technical Package. This is a 100% grant-funded position serving the State of New York, focusing primarily Brooklyn, Queens, Manhattan, and the Bronx. Location: This position is located at the American Lung Association's New York, NY office and will be a hybrid of in-person and virtual work. Responsibilities: • Design, implement, evaluate, and monitor evidence-based initiatives for the Addressing the Burden of Childhood Asthma in NYS grant by establishing and managing work plan projects that address the needs of the high-risk population incorporating the EXHALE Technical Package., • Manage and deliver Project BREATHE NY to members of the multidisciplinary asthma care team across many healthcare settings. Ensure program quality, performance and effectiveness., • Develop, maintain, and disseminate asthma care delivery tools to guide implementation of guidelines-based care., • Collaborate with other key staff members, partners, and organizations working to expand comprehensive asthma control services., • Work with national and local advocacy team to promote asthma as a priority., • Develop and submit timely reports as required by the association and funding sources, including monthly management reports, grant reports and program metrics reports., • Present program outcomes and disseminate surveillance/ evaluation findings at statewide and national meetings as needed., • Represent the American Lung Association as a member of community, civic, and/or health coalitions and organizations related to the Association’s mission., • Build, maintain, and cultivate relationships with funders, facilitators, and community partners to encourage program delivery, sustainability, and growth across service territory, including American Lung Association programs where applicable., • Select and participate in local, state, and national seminars and courses designed to increase skills and knowledge related to job requirements., • Participate in and support all area events and provide support for annual report, awards, grants and other activities as assigned., • Bachelor’s degree in public health, Health Promotion, or related field required. Master’s degree in public health, or a related field preferred., • Minimum of three to five years of experience developing and implementing community awareness, education, stakeholder engagement and programs specifically related to areas of public health., • Certified Asthma Educator (AE-C), Respiratory Therapist (RRT), Registered Nurse license (RN) or Certified Health Education Specialist (CHES) preferred; must obtain AE-C within one year of hire. Prior experience in public health, social services, public policy, and/or advocacy required., • Experience in coalition building, developing and implementing community awareness, education, and programs specifically related to areas of public health., • Experience in the management, implementation and evaluation of programs., • Must be a self-starter with excellent communication skills, both written and oral., • Positive attitude with the ability to work independently and cooperatively in a team environment., • Must have a valid Driver’s license and your own reliable transportation with the ability to travel within assigned area 60% of the time for meetings and conferences, as well as the flexibility to work irregular hours, including evenings and weekends with some overnights required., • Able to work with minimum direct supervision, make decisions, and take initiative., • Proven ability to cultivate and steward relationships across a diverse population., • Must be proficient in Microsoft Office and Internet applications., • Ability to lift approximately 25 lbs.
