Job Description POSITION TITLE: Registered Dietician REPORTS TO: AVP of Pediatric Wellness DATE REVISED: August 5th, 2025 UNION OVERVIEW Union Community Health Center (UNION), operating continuously as a health care provider in the Bronx since 1909, is one of the largest Federally Qualified Health Center (FQHC) in NYS, with a proven record of accomplishment of providing a full range of quality primary care and dental services for children, adolescents, adults, and seniors of underserved, high-need communities throughout the Bronx. UNION’s mission is to provide comprehensive, culturally competent primary and preventive health care services to improve the health status and decrease health disparities of the medically underserved populations in the Bronx, providing services to all regardless of age, race, ethnicity, sexual orientation, religion or the ability to pay. UNION continues to impact more residents each year with five clinic locations and two Mobile Units (medical and dental). UNION serves over 30,000 underserved residents of the Bronx provides over 180,000 patient care visits annually. UNION is an impressive model of a holistic and integrated approach to patient care that unifies primary care, behavioral health, and dental care providers, creating one team, designing and delivering care for each patient. Additionally, UNION provides specialty services, needed and otherwise difficult for Medicaid patients to access, including physical and occupational therapy, speech therapy, audiology, cardiology, behavioral health and psychiatric services among many others. PRINCIPAL MISSION: Under the direct supervision of the AVP of Pediatric Wellness, the Registered Dietitian plans, directs and provides nutritional care to patients conducts nutritional assessments and provides nutrition education for patients and members of the medical team. Responsibilities include providing appropriate Nutritional Care for the age populations served (infant, pediatric, adolescent, adult, and geriatric). PRINCIPAL DUTIES AND RESPONSIBILITIES: • Promote a safe, cooperative and professional health care environment to ensure optimal p[patient care., • Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct., • Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner exercising courtesy and tact. Effectively communicates with patients and others about UCHC services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in UCHC in a timely fashion,, • Interviews patients, families, and/or significant others to obtain diet history necessary in planning the patient's diet,, • Gathers appropriate anthropometric, biochemical, and physical data necessary in assessing patients' nutritional status and documents information accurate in the medical record., • Assesses patients' nutritional status based on standards of nutritional care and passing in a rate level of nutritional risk., • Develops appropriate care plans based on standards of care, including recommending rate diet or nutrition support,, • Reassesses patients based on level of nutritional risk or change in condition and modifies the plan of care as necessary., • Collaborates with members of the health care team on a regular basis in developing and implementing treatment plans and discussions., • Assesses patient/family/significant others' educational needs, readiness to learn, and barriers to learning; provides the education and documents information in the medical record., • Keeps current in the field of nutrition through continuing education to maintain registration status through the American Dietetic Association (75 Continuing Education Units/5-year period). Completes 3 CELI annually in area of practice and 3 CELI related to age specific topics (pediatric, geriatric, • Demonstrates competency in assessing the nutritional needs of patients throughout the lives of pediatric, adult, and geriatric., • Performs all other duties as directed*, • Conducts nutrition education classes for patients and staff as needed., • Functions as a preceptor for dietetic intern students in areas of clinical and community nutrition and evaluates student’s progress., • Communicates regularly with Chief Clinical Dietitian regarding patient care and operational issues., • Counsel patients/families on medical nutrition/diet therapy principles and restrictions., • Utilize available medical nutrition therapy education materials or obtain from reputable sources, • Document the nutrition counseling session in the Electronic Health Record., • Assist in education to increase patient referrals as needed. QUALIFICATIONS: • Education Required, • B.S. in Food and Nutrition / Dietetics, • License(s) Required, • Registered Dietitian or Registry Eligible, • Experience/Skills Required, • Two (2) years' experience in Clinical Dietetics. Ability to speak Spanish desired but not required. Variable from patient areas; may involve walking.
