Job Description Internal Medicine Provider Salary: $200,000+/Year based on experience LaSante Health Center is seeking an Internal Medicine Provider to join our multispecialty, community-based health center in Brooklyn. The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Creole-speaking candidates are strongly preferred. Responsibilities: • Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management, • Diagnose and treat acute and chronic medical conditions, • Collaborate with multidisciplinary teams to coordinate patient-centered care, • Counsel patients on lifestyle modification, disease prevention, and medication management, • Maintain accurate, timely documentation in the electronic health record (EHR), • Participate in quality improvement initiatives and team meetings, • Medical degree (MD, DO, NP, or PA) from an accredited institution, • Board certification or eligibility in Internal Medicine or Adult Primary Care, • Current New York State license or eligibility for licensure, • Experience in community health or outpatient primary care preferred, • Excellent communication and interpersonal skills, • Bilingual Creole/English strongly preferredCompany DescriptionLaSante Health Center is a federally qualified, multispecialty practice in Prospect Lefferts Gardens, Brooklyn, serving Flatbush, East Flatbush, Crown Heights, Park Slope, Little Haiti, and Little Caribbean.
