Job Description Job Title: Reach One Coordinator of Activities Reports To: Asst. Director of Rehab and Recovery. Program: Rehabilitative and Recovery Services Location: Valley Cottage, NY Hours Per Week: 35 FLSA Status: Exempt Salary Range: 25 per hr Summary Description: The Reach One Activities Coordinator is a certified Peer Professional (NYCPS and/or CRPA) responsible for overseeing the day-to-day operations of the Reach One events and activities. This role includes supervising Peer Professional interns; managing program activities, and facilitating Peer Support groups weekly while maintaining an active client caseload. Responsibilities: Supervision & Training • Supervise and train Peer Professional Interns working in Reach One and related programing., • Provide ongoing feedback, coaching, and performance support to ensure professional growth and adherence to best practices. Group, Event and Activity Oversight • Oversee daily planning and facilitation of groups, activities and events for Reach One Participants., • Assist Reach One staff and interns in planning additional groups, events, and activities., • Track expenses, attendance and other key data points for Reach One programing., • Collaborate with leadership and stakeholders to develop, implement, and evaluate program policies and procedures., • Develop unique programing, in collaboration with Reach One participants, staff and interns. Leadership & Meetings • Promote a supportive, collaborative, and recovery-oriented team culture., • Facilitate meetings, as needed, for Reach One staff and interns. Group Facilitation • Plan, facilitate, and evaluate therapeutic Peer Support groups using evidence-based approaches., • Foster participant engagement, empowerment, and recovery skills. Peer Support • Maintain an active caseload providing individual counseling and support., • Provide Peer Support, Transportation Training, social/community engagement, employment/educational support, ADLs, housing assistance., • Develop, implement, and monitor treatment plans aligned with member goals. Documentation & Compliance • Complete all documentation accurately and within required timeframes., • Ensure compliance with program, agency, state, and regulatory standards. Advocacy & Collaboration • Advocate for member needs and rights within the program and community., • Collaborate with multidisciplinary teams to promote integrated care., • Connect members with appropriate community resources and supports. Engagement, Networking, and Marketing • Build and maintain relationships with local referral partners., • Connect with potential referral partners to ensure they are aware Reach One Services., • Support Referral partners to complete referrals to Reach One., • Engage potential Reach One participants and support them into the program as appropriate., • Build Program engagement by identifying new potential clients, referral sources and areas of growth. Competencies: • Certification as a Peer Professional (CRPA and/or CRPA), or ability to complete certification within three months of hire., • Previous experience and/or training in Peer Professional techniques, ethics and practices., • Demonstrated supervisory or training experience preferred., • Experience developing and/or facilitating creative programing strongly preferred., • Excellent leadership, communication, and organizational skills., • Empathy, active listening, and strong crisis intervention skills., • Commitment to recovery-oriented, trauma-informed, and culturally competent care., • Collaborative and adaptable in a fast-paced team environment. Key Performance Indicators (KPIs): • Program Enrollment (Reach One), • Attendance of Groups, Events, Activities., • Satisfaction of Reach One Participants., • Reduction of ER use and/or Hospitalizations., • Appropriate and timely documentation of charting of a full caseload of Reach One participants. Requirements: • Certification as a Peer Professional (CRPA and/or CRPA), or ability to complete certification within three months of hire., • Experience with group, events, activity development and facilitation., • Must maintain current NYS Defensive Driving certification within 30 days of hire., • Solid computer knowledge: typing, communication tools, scheduling tools and web browsers., • Proficiency in Microsoft Office suite and database management systems., • Fluency in English (Reading, Writing, Speaking)., • Must be cleared and maintain clearance by applicable regulatory clearances as required., • Legally eligible to work in the United States without sponsorship. Special Considerations: • Must maintain confidentiality and professional boundaries., • Requires flexibility, adaptability, and strong organizational skills., • May involve hybrid or field-based work as needed., • Hours per week, reporting structure, and location are subject to change per program needs.
