Job Description Speech Language Pathologist, SLP River City Health Care Center – Jacksonville, FL Department: In-House Subacute Rehabilitation Schedule: Full Time About Us River City Health Care Center is a dedicated skilled nursing and rehabilitation community serving residents in the Jacksonville area. Our in-house subacute rehabilitation team provides high-quality, person-centered therapy services to help residents regain independence and improve quality of life. Position Summary We are seeking a compassionate and motivated Speech-Language Pathologist (SLP) to join our in-house rehab team. The SLP will evaluate, diagnose, and treat residents with communication, cognitive-linguistic, and swallowing disorders within our subacute rehabilitation setting. Responsibilities • Conduct comprehensive speech, language, cognitive, and swallowing evaluations., • Develop and implement individualized treatment plans to target communication, cognition, and dysphagia needs., • Provide evidence-based, resident-centered therapy services., • Document evaluations, daily treatment notes, progress reports, and discharge summaries following facility policies and regulatory requirements., • Collaborate with an interdisciplinary team—including PT, OT, nursing, dietary, and medical staff—to ensure coordinated, holistic care., • Educate residents, families, and caregivers on therapy goals, strategies, and recommendations., • Participate in care conferences and interdisciplinary meetings., • Maintain compliance with Medicare, state, and federal regulations, as well as ASHA professional and ethical standards., • Active Florida Speech-Language Pathology license (or eligibility for licensure)., • Master’s degree in Speech-Language Pathology from an accredited program., • Certificate of Clinical Competence (CCC-SLP) preferred; CFs considered with appropriate supervision available., • Experience in skilled nursing or subacute rehabilitation preferred., • Strong clinical skills in dysphagia management, cognitive-linguistic therapy, and communication disorders., • Excellent communication, documentation, and teamwork skills., • Commitment to providing compassionate, high-quality care to older adults. #IND1
