Job Description Overview The Work: A Speech Pathologist evaluates and treats patients per physician order utilizing a variety of therapeutic techniques. Speech Pathologist communicates with families, physicians, and other health team members regarding residents, maintains timely documentation in each medical record, laboratory, and service logs as well as participates in rehab team meetings and appropriate committees. This position performs all duties and responsibilities in accordance with the Mission, Vision, and Core Values of Cayuse. Flexible Contract Opportunity – 1099 Independent Contractor We are seeking providers for a flexible contract opportunity with zero long-term commitment. Here's what you can expect: • Advance Scheduling: Coverage requests will be communicated well in advance. There will be no last-minute, same-day phone calls or rush requests. You will have ample time to review and determine your availability for the requested dates or periods of work. Responsibilities Key Responsibilities: • Determines each patient's need for skilled therapy based on objective and documented criteria, • Evaluates patients within established protocol of physician's order. Conducts the evaluation according to the standardized or recommend techniques and procedures including timely documentation., • Develops treatment plan at the end of the evaluation which includes measurable and objective long and short term goals for each patient based on the evaluation data. Follows each treatment plan in an objective, measurable and documented approach, • Prepares a daily work schedule and completes all treatments scheduled, • Requests assistance or adjusts therapy scheduled to avoid interference with the quality of service, • Schedule Residents for treatment after receipt of the written referral, • Provide documentation, rehab screenings to Residents newly admitted to AFRH-W and returning from outside hospital stay, evaluations, treatment plans, equipment recommendations, evidence based best therapeutic treatment practices, and initial, progress, and discharge notes, in compliance with all CARF and AFRH Health Care and Rehab services policies and SOPs, • Evaluate and diagnose speech, language, communication, and swallowing disorders., • Treat speech, language, communication, and swallowing disorders., • Provide training and education to family/caregivers and other professionals., • INFORMATION SHARING: Sharing of Resident medical information (inclusive of electronic medical records) at the facility shall follow all Health Insurance Portability and Accountability (HIPAA) regulations., • Other duties as assigned. Qualifications Qualifications – Here’s What You Need: • Master’s degree, which included 18 semester hours in the field of speech pathology with approval clinical practice., • Two (2) years of professional speech therapy experience, in the field of geriatrics., • The following documents are required for credentialing the speech pathologists:, • Current License registered in DC, • Certification registered in DC, • Professional School attended (to include diplomas), • Declaration of Health, • Relevant Training or Experience, • Initial Application for Clinic Privileges, • Delineation of Privileges, • Statement of Application, • Privacy Act Statement, • Curriculum Vitae, • CPR Certification (BLS), • Continuing Education Certificates, • National Criminal Background Check, • Drug Screening, • PPD and Vaccination Record (Influenza and COVID-19), • Td/TdaP (up to date), • Must be able to perform job duties in person and over the telephone., • Must be able to communicate clearly to patients in person and over the telephone., • Must possess problem-solving skills., • Exceptional communication skills, both oral and written, • Ability to respond effectively to customers with a sense of urgency., • Proficient in Microsoft and Adobe toolsets, including Excel, Word, PowerPoint, Acrobat, etc., • Highly motivated with the ability to handle and manage multiple tasks at any one time., • Ability to forge new relationships, individual and teaming in nature. Working Conditions: • Professional Walk-in medical clinic with high patient volume environment., • Must be physically and mentally able to perform duties while standing for extended periods of time., • Ability to use a computer and other office productivity tools with sufficient speed to meet the demands of this position., • Must be able to establish a productive and professional workspace., • Must be able to sit for long periods of time looking at computer screen., • May be asked to work a flexible schedule which may include holidays., • May be asked to travel for business or professional development purposes., • May be asked to work hours outside of normal business hours., • Visual acuity adequate to perform job duties, including visual examination of patient and reading information from printed sources and computer screens., • Requires frequent lifting and carrying items weighing up to 30 pounds unassisted, including assisting patients when required., • Adequate physical ability including sufficient manual dexterity to perform the requisite health care service. Requires frequent bending, reaching, repetitive hand movements, standing, walking, squatting and sitting, with some heavy lifting, pushing and pulling exerted regularly throughout a regular work shift.
