The Referral Coordinator ensures all referrals are tracked, followed up, and closed in accordance with ACHCs Referral Tracking and Follow-Up Policy. This includes verifying that patients attended specialist appointments and that consult reports are received, reviewed, and filed to ensure continuity of care (closing the loop). Apex Community Health Center also requires the Referral Coordinator to participate in trainings and quality improvement activities. ESSENTIAL DUTIES Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety and satisfaction. Ensure complete and accurate registration, including verification of demographics and current insurance information. Assemble information concerning the patients clinical background and referral needs. Contact review organizations and insurance companies to ensure prior approval requirements are met. Review details and expectations about the referral with patients. Assist patients in problem-solving potential issues related to the health care system, problems with insurance coverage, or social barriers such as interpretation or transportation needs. Identify and utilize community and cultural resources suited to patient populations. Track all referrals in ECW (EClinicalWorks) and referral tracking log from initiation to closure, ensuring timely follow-up and documentation. Monitor referral timelines and ensure completion based on urgency (urgent: 2472 hours; routine: 72 hours2 weeks). Follow referral closure protocol: verify that patients completed the referral visit, confirm Apex received consult reports, and document outcomes in ECW and tracking log. If consult reports are not received within three weeks of appointment, follow up with the specialists office and document contact attempts. Close referrals within 30 days of appointment or after three unsuccessful contact attempts (two phone calls and one letter). Immediately notify the ordering provider of any critical or time-sensitive findings received from specialists (same day or within 24 hours). Ensure consult reports are reviewed by providers, scanned into the patients EHR, and that care plans are updated accordingly. Ensure referrals are addressed in a timely manner. Participate in initial and annual compliance and referral tracking training per HRSA/PCMH standards. Assist in preparing referral data and reports for Quality Improvement Committee quarterly review. Cross-train as a back-up in the dispensary in case the pharmacy technician is absent. Must demonstrate integrity, sound judgment, leadership skills, and strong interpersonal skills. What we offer (We like to get right to it!) • $300 for part-time employees after 150 clinical hours worked within 6 months, • $2/hour for weekend shifts, • $5/hour for company-observed holidays, • Monday-Friday between the hours of 2:00pm – 8:00pm, • Cities Targeting- Woodland Hills, Sylmar, Pacoima, Van Nuys, North Hills, Northridge, Canoga Park, Granada Hills, • We aim to match you with clients near your home, and we offer mileage reimbursement if your first client is over 20 miles away, • Student loan repayment assistance for eligible roles, • Bonus program(s) for eligible roles, • Career development and advancement opportunities, • Flexible scheduling, • Great and fun company culture, • Expansive Health, Vision, and Dental plans for our full-time partners, • 401(K) retirement savings program, • Mileage and phone reimbursement, • Provide 1:1 care directly with Clients during scheduled sessions under the supervision of a Board- Certified Behavior Analyst (BCBA)., • Implement established behavioral treatment plans based on the science of applied behavior analysis (ABA) across skill levels, moderate to severe problem behaviors, and service delivery settings., • Ensure that 100% of the scheduled direct services are provided without any scheduling interruptions., • Maintain accurate and timely documentation of client progress, interventions, and outcomes., • Collect data related to Client’s progress during treatment sessions., • Incorporate activities and games that are developmentally appropriate and engaging for the client in alignment with the treatment plan., • Promote Client engagement to build their strengths in a natural and enjoyable manner., • Consulting with case Supervisor on treatment plans, Client updates, and intervention strategies., • Receiving and utilizing ongoing training, guidance, and support., • Follow program procedures, administrative tasks, direct therapy skills, and other duties that are in line with our compliance requirements., • Maintaining reliable communication to record scheduled appointments and return phone calls within 24 hours., • Ensuring that the company’s “zero tolerance policy” is understood, implemented, and reinforced in the region., • Completing all duties, tasks, and/or special projects assigned by leaders and/or the business., • Participating in team meetings, trainings, and professional development opportunities., • Providing make up hours as an alternate care team member., • High School Diploma or GED (minimum), • Spanish Bilingual Required, • Reliable Transportation, • Successfully complete Live Scan background check, employment verification and TB screening, • Be able to lift up to 30lbs.
