Serves as a role model, advocate, and mentor for children and families, • Assists with obtaining and maintaining clients’ entitlements, including Medicaid, Public Assistance, SSI as well as assisting with problems related to housing or utilities, • Interviews clients and significant others, when appropriate, in order to obtain personal histories and to understand the social and emotional factors related to the client’s mental health problems, • Participates in frequent active engagement with families to engage them in receiving services and assists in empowering them to take the steps necessary to maximize their quality of life, • Develops inventory of resources that will meet the clients’ needs as identified in the assessment process, • Advocates for and assists clients in utilizing adjunctive community resources. Arranges transportation and accompanies clients to appropriate facilities/agencies, as necessary, • Provides linkage, coordination with, referral to and follow-up with other service providers. Job Description Position Overview: Braveheart’s staff are comprised of young professionals who are often referred to as peer navigators, credible messengers and advocates. These individuals harness various levels of lived experience in the NYS child welfare and juvenile justice system of services for youth/young adults with high end needs. The Braveheart Peer Navigators utilize their unique skill set to authentically connect with youth participants in an effort to create plans for sustainable and realistic community connectedness for those youth who are the most at risk of arrest, homelessness and social disconnection. This is achieved by giving youth voice a meaningful platform and establishing the value of partnership with existing professional supports already identified by the client while uplifting the voices and causes of youth. The Bravehearts Peer Justice Advocate will provide mentoring, advocacy, group facilitation and resource connection to young adults adversely impacted by youth serving justice systems. The Peer Justice Advocate will provide programming on Children’s Village campus to youth who exhibit runaway and high-risk behaviors requiring court activity through the Raise The Age initiative as well as provide programming in the county correctional facility and juvenile detention center. The Peer Justice Advocate will provide advocacy with and on behalf of the youth participant by working in partnership with professionals and systems that the youth is impacted by. The Embedded Peer Specialist provides peer support for each participant on the Rockland County Psychiatric Unit and in the Emergency Department. The Peer is available to instill hope, support wellness and demonstrate that recovery is real and does happen. Peers model and utilize trauma-informed techniques and tools, help participants set goals and engage in community and social events.Duties and Responsibilities: Provide support for participants to divert hospitalization after discharge. Share personal experience and encourage mental health wellness and recovery. Assist in connection with mental health services, community resources, entitlements, and other services as needed.
