Essential Duties and Responsibilities • Maintains a caseload of clients form the STEHP program, • Conducting intake assessment and services to clients referred to STEHP from the shelters, self-referrals and or partnering agencies., • Completes required intake paperwork and any relevant screenings., • Complete all client’s paperwork and goals into the HMIS system upon initial intake, • Makes referrals to housing, DHS, education, and civil legal issues regarding evictions., • Complete letters of requests/referral forms, applications, and other required documents of eligibility to other agencies and or organization to advocate for resources which the client is initiated to., • Guide clients to complete applications for financial assistance, finding the best fit among community funders based on the client’s needs, goals, and abilities. Assist the client in understanding and meeting expectations of service providers, courts, and employers. May deliver instructions in various topics that support client’s ability to live independently. Submits required reports and statistics in a timely manner. Collaborates effectively with program partners to heighten efficiency of joint efforts across multiple organizations Communicates with appropriate program partners, agency staff, community, and criminal justice agencies to assure ongoing and effective referrals into programs. Serves as Case Coordinator for clients for program care. Works with the Nurse, Peer, and other stakeholders to ensure clients’ needs are being met. Coordinates and participates in regular case conferences. Monitors and ensures achievement of program milestones and client outcomes. Oversees client recordkeeping in compliance with appropriate regulatory agencies. Reports to service providers about significant individual behavior or events that will require an immediate response. Acquire concrete resources to meet individual needs. Complete letters of requests/referral forms, and other required documents of eligibility or charitable organizations and public agencies to advocate for resources that the client is entitled or eligible for. Makes referrals for education, employment, legal issues, and others as needed. Document need for financial assistance or other resources such as wrap around from the project to stabilize client finances. Utilizing various databases and/or on-line platforms, maintains all documentation in accord with the agency and appropriate regulatory standards. Informs supervisor of significant events / issues related to clients as appropriate and follows program’s defined incident reporting process as required. Attends regular program and departmental meetings, case review, and CFC quarterly meetings. Maintains and develops professional knowledge by completing recommended education and training, workshops, conference attendance, and readings. Maintains knowledge of regulations and standards that govern client population. Participates in appropriate interagency meetings with other treatment agencies and community groups as required. Job Description Role and Responsibilities Provide collaborative, client-centered support to Health Home Program clients using the development of person-centered goals, culturally competent care management, and professional healthcare and social service coordination. Health Home Care Managers will evaluate, manage, and integrate solutions and resources for all primary, complex chronic diseases, behavioral health and long-term care needs in the Health Home Program. This position is designated as a higher-skilled Care Manager, capable of exceeding the basic tenants of care management. Essential Functions • Actively and progressively care manage an enrolled client caseload as determined by Agency guidelines. Develop an individualized plan of care with specific goals/interventions/objectives, to be revised as needed., • Provide rehabilitative and supportive counseling geared toward the restoration of clients to their optimum level of social and health functioning.
