Work Schedule: Full-time/40 Hours Position Type: On-site Branch Location: Westbury, NY This is NOT a remote or work-from-home position. This position requires you to sit on-site at our Westbury, NY branch location. The RN Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel. This role is a focused on both home health clinical quality assurance and home health clinical operations initiatives. * Develops, plans, implements, analyzes, and organizes clinical operations for a specific location managed. * Conducts/delegates the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies. * Manages the assignment of caregivers. * Responsible for and oversees the delivery of care to all patients served by the location. Receives case referrals. Reviews available patient information related to the case, including disciplines required, to determine home health or hospice needs. Accountable to ensure patients meet admission criteria and make the decision to admit patients to service. Assigns appropriate clinicians to a case, as needed. * Instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate. * Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing patient Plan of Care (POC). * Monitors cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers. Ensures final audits/billing are completed timely and in compliance with Medicare regulations. * Coordinates communication between team members/attending physicians/caregivers to ensure the appropriateness of care and outcome planning. * Works in conjunction with the Branch Director and Company Finance Department to establish location’s revenue and budget goals. * Participates in sales and marketing initiatives. * Supervises all clinical employees assigned to a specific location. Responsible for the overall direction, coordination, and evaluation of the location. Carries out supervisory responsibilities in accordance with Company policies and procedures. * Handles necessary employee corrective action and discipline issues fairly and objectively, in consultation with the Human Resources Department and the Executive Director/Director of Operations. * Participates in the interviewing, hiring, training, and development of direct care clinicians. Evaluates their performance relative to job goals and requirements. Coaches staff and recommends in-service education programs, when needed. Ensures adherence to internal policies and standards. * Assesses staff education needs based on own the review of clinical documentation in addition to feedback and recommendations by Utilization Review staff. Upon completion of the assessment, creates and conducts regular staff education as needed. * Analyzes situations, identifies problems, identifies and evaluates alternative courses of action through the utilization of Performance Improvement principles. * Responsible for review of the appropriate number of Case Managers and clinical staff documentation to include starts-of-care, resumption-of-cares, and re-certifications, for appropriateness of care, delivery, and documentation requirements. * Responsible for the QA/PI activities. Works with Utilization Review staff relative to data tracking for performance review and outcomes of care analysis to determine efficiency, the efficacy of case management system as well as any other systems and process. Competently performs patient care assignments and staff management activities. * Provides direct patient care on an infrequent basis and only in times of emergency. * Acts as Branch Director in his/her absence. * Interprets Company standards and Company policies and procedures to ensure compliance with external regulatory authorities and ensures that caregiver clinical documentation meets internal standards. * Participates in performance improvement activities, maintains ongoing clinical knowledge through internal and external training programs. Provides interpretation of knowledge and direction to staff. * Maintains relationships with referral/community sources. Participates in professional organizations and conducts care-related programs. * Performs other related duties as assigned or requested. Required Experience/Skills: * Graduate of an accredited School of Nursing. * Current state license as a Registered Nurse. * Proof of current CPR. * Valid driver’s license, auto insurance and reliable transportation. * Two years as a Registered Nurse with at least one-year of management experience in a home health, hospice or equivalent environment. * Certified Home health Agency experience is required (NY state requirement) or four years' CHHA experience and six credit hours toward BSN. * Management and people leadership experience, preferred. * OASIS experience, required. OASIS certification (HCS-O, COQS, and/or COS-C), preferred. * Homecare Homebase (HCHB) experience, preferred. * CMS PDGM billing knowledge and/or experience, preferred. The Regional Account Director is responsible for developing and managing all aspects of the relationship with assigned accounts, including regional commercial payers, PBMs and government payers. The Behavioral Health Utilization Management (BH UM) Clinician is responsible for conducting utilization and quality management activities in accordance with New York State and MetroPlusHealth Utilization Management policies and procedures. Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City’s efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient’s clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. Duties & Responsibilities Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. The Unit Chief is the mental health administrative lead in each facility or designated area, including general population and therapeutic housing units. Under the direction of the Director of Mental Health Administration, this full-time position coordinates, and helps manage, mental healthcare delivery in one or several of the jail facilities in New York City. This includes ensuring provision of services within the timeframes and quality benchmarks outlines by CHS MH policies, providing administrative support to staff who are assigned to their clinical area, maintenance of patient records and compliance with monitoring requirements. S/he will act as the principle administrative liaison between mental health disciplines, as well as pharmacy, medical and nursing. As a member of the facility management team, the Unit Chief will collaborate closely with the Department of Correction (DOC) leadership and CHS Operations in order to ensure service delivery and meet patient needs. A major component of this position will be providing effective, inspiring, and clinically-informed administrative leadership and organization. Position Summary: The Senior Director is accountable for the strategic planning, communication, organization, and execution of Specialty Pharmacy operations and initiatives in alignment with the Medical Center’s mission and vision. The Senior Director oversees all Specialty Pharmacy functions related to the procurement, distribution, monitoring, and utilization of pharmaceuticals. In partnership with the Medical Staff and the Pharmacy and Therapeutics Committee, the position develops systems and services that promote optimal therapeutic drug use for patients. The role is also responsible for analyzing and managing departmental financial performance and ensuring the quality of all services and activities across the continuum of care. Additionally, this position provides consultative expertise on the Profession and Practice of Pharmacy to health‑system affiliates. The Senior Director will collaborate closely with Shared Services and offer subject‑matter guidance on pharmacy‑related contracting. Essential Functions: Develops and approves mission and goals for comprehensive pharmaceutical services and provide high quality comprehensive pharmaceutical services to meet the needs of the patient and standards of accreditation. Monitors quality of services and evaluate new technology in pharmaceutical systems improving patient outcomes. Managing the pharmaceutical supply chain. Oversight of the pharmaceutical contracting, procurement, receiving, security, inventory control, continuum, including outsourced sterile products, alternative distribution channels used for drug shortages, reverse distribution and other methods of pharmaceutical waster disposal. Assuring quality outcomes through performance-improvement activities leveraging pharmacy expertise in support of value-based purchasing, including leading core measures initiatives involving medication therapy. Plays an active role in reducing readmissions and owning the process for medication-related customer satisfaction indicators. Identifies and implements specific ways that the pharmacy enterprise can contribute to the patient's experience. Optimizes the use of information systems and technology. Provides leadership at the organizational level regarding planning, purchasing, implementing and maintaining information systems that support patient care. Commits to achieving patient safety innovations made feasible by electronic health records and other clinical applications. Ensures compliance with regulatory and accreditation requirements. Continues compliance with all national, state and local regulations related to medications and their management. Fulfills all requirements of the State Board of Pharmacy, DEA, CMS, TJC and other medication management accreditation standards. Qualifications: Pharm D is required. Must have valid NYS Pharmacist license and registration with ten years+ of working experience. SUPERVISED BY: On Site Director of Nursing Deputy Director of Nursing GENERAL DUTIES: The Patient Care Coordinator (PCC)/Supervisor of Nurses (SON diagnosis and treats human responses to actual or potential health problems through such services as data collection, physical assessment, case-finding, health teaching, health counseling and provision of care supportive to /or restorative of life and well-being as data collection, physical assessment, health teaching, health counseling, and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician, dentist or other health care provider legally authorized under this title and in accordance with the commissioner’s regulation. Under the guidance of the On-Site Director of Nursing (OS-DON), the Patient Care Coordinator/Supervisor of Nurses is responsible for directing and administering nursing care to patients. He/she coordinates activities through collaboration with physicians, administration, pharmacy, clerical, and correctional staff, etc. He/she is guided by a thorough knowledge of principles of nursing based on biological, physical, and psychosocial sciences. He/she possesses a working knowledge of principles of leadership and utilizes that knowledge in guiding and directing nursing staff. He/she assists the OS- DON in ensuring compliance with standards, policies and procedures of NYC Health and Hospital Corporation Correctional Health Services contracted with Prison Health Services, Inc. and the Department of Nursing. He/she will be working with adult male and females between sixteen to eighty and infants from birth to one year of age. RESPONSIBILITIES: • Provides direct patient care as necessary. Using the problem-oriented progress notes, written to match each problem documented on the Problem List and organized using the SOAP format., • Coordinates the nursing care provided for patients within a given unit by:, • Assigning appropriate nursing staff for patient care activities such as admissions, sick call, treatments, medications, punitive segregation rounds, follow-up, etc. Problems in staffing must be communicated to the On-Site Director of Nursing., • Utilize therapeutic communication skills and effective interviewing techniques to elicit pertinent historical information and to make appropriate referrals as required. Screen patients with mental health problems in an immediate manner and makes appropriate referral as required., • Making rounds to patient care areas for the purposes of assessing, planning, implementing, documenting, and evaluating unit nursing care delivery. Assist OS-DON in staffing needs. Provides guidance and assistance to nursing personnel in the delivery of patient care., • Assuring continuity of care on other shifts through written and/or verbal communication. Communicates special care needs of particular patients to the psychiatrist, On-Site Director of Nursing or the On-Island Administrator., • Collaborates with the OS-DON in the orientation of new staff on the unit.
