Description Position Overview: The Life Coach at Lee Goodwin Residence provides comprehensive case management, outreach, and counseling to the residents at our supportive housing facility for young adults aging out of foster care between the ages of 18-28 year olds. The Life Skills Coach will provide referrals to: education and career programs such as: HSE, Vocational Training, Employment, and Internships. As well as: Substance Abuse Treatment, and Mental Health Services. LSA Recovery, Inc. is an 822 Medically Supervised Outpatient Chemical Dependency Program, which provides a variety of services to individuals as well as family members seeking help with substance abuse and chemical dependence problems. We believe that chemical dependence treatment requires a detailed knowledge of the history and personality of each patient. This is accomplished by professional and knowledgeable staff that will conduct a complete and thorough interview with the patient and his/her family, as well as contact with their primary care physician. The data is then evaluated by various professional staff members in order to develop a personalized treatment plan. We organize the plan of care to enable the patient to proceed through the recovery process with a support network, increase their self-esteem, and understand the effects of chemical dependence. Summary: The housing Specialist is responsible for assisting and supporting clients in housing resources, benefits, and placement through the entire treatment experience from admission to discharge; Connects and engages the client to housing resources and support services to aid continuity of care and self-sufficiency post-discharge, monitors the course of treatment for each client in a pro-active manner to minimize the barriers that might compromise the client's treatment and post-treatment living environment. Assist client with creating an individualized housing case management plan that will outline the client's goals and housing objectives, as part of the multidisciplinary team.
