SUMMARY Under general supervision, the Community Habilitation Specialist provides individualized training and personal assistance to participants with developmental and/or physical disabilities in their homes and communities. The goal of this position is to promote meaningful independence, personal growth, and full community inclusion. Specialists work with minimal supervision to deliver skills training, role modeling, structured routines, and specialized supports as outlined in each participant’s Community Habilitation Plan. Activities and routines are individualized and based on participant choice, emphasizing empowerment, dignity, and self-determination. Job Description Position Description Outreach Specialist Outreach Specialist is responsible for providing an array of in-house and mobile-based harm reduction services to individuals in the target population, including veterans, people experiencing homelessness, people who actively use and inject drugs, people living with HIV or Hepatitis C, people who engage in sex work, and other high-risk populations. This position provides safer use supplies, personal care items and pantry, health education, and assistance with referrals. Overdose prevention education is conducted on a regular basis to all clients with a history of substance use. The Outreach Specialist will work with patient navigation staff to link individuals to additional services. The Outreach Specialist will also work to collaborate with other veteran-serving organizations to connect individuals with substance use and harm reduction services, provide additional opportunities to connect with supportive care, and to ensure access to needed medical assistance.
