Job DescriptionDescription: COMHAR's Intensive Behavioral Health Services (IBHS) is located in the Philadelphia are. IBHS support children, youth, and young adults with mental, emotional, and behavioral health needs. Services are provide to children, youth, and young adults across all settings (at the home, school, and community settings) in order to both help them reduce and/or replace problem behaviors with more positive, socially appropriate behaviors, with the highest priority placed on family engagement throughout the process. COMHAR’s Intensive Behavioral Health Services (IBHS) has an immediate opening for an Licensed Clinical Supervisor. Licensed Clinical Supervisor Salary: $62,000.00-$65,000.00 Licensed Clinical Supervisor Scheduled: Monday-Friday 8:00AM-4:00PM The License IBHS Clinical Supervisor is responsible for assisting the Clinical Coordinator in ensuring the overall quality of clinical services and documentation of services provided in Children’s Services Department. This includes the following: • Chart monitoring to ensure that treatment plans, progress summaries, discharge summaries are in line with the Network Inclusion Criteria Standards for Excellence., • Providing feedback and training to clinicians and Behavioral Health Technicians while reviewing areas of needed improvement., • Track and monitor the completion of outstanding paperwork i.e. Tx plans., • Review with Care Coordinators issues that may have led to services being reduced or denied., • Assignment or re-assignment of cases., • Complete and update treatment plan tracking list, along with distribution of list to clinicians, • Complete initial treatment plans Ensuring that clinicians perform the following: • Engage individuals who may be at-risk and provide age-appropriate education and supports., • Assist in the orientation process for persons who are new to receiving mental health services, • Assess individuals in their abilities, strengths, and assets towards the development of a plan that addresses all domains of an individual’s life., • Assist individuals in identifying their personal interests and goals using motivational interviewing. Clinicians will continue to utilize goal sheets to ensure that clients have a strong understanding of treatment planning process. Provide individual and group counseling, mentoring instruction, training, feedback and positive reinforcement for individuals in areas such as conflict resolution, problem-solving and anger-management., • Educate individuals on evidenced based practices. Ensuring that Clinicians and Behavioral Health Technicians perform the following: • Assist and support individuals in utilizing educational and support groups, activities deemed beneficial for clients., • Work closely with school, camp and community organizations in developing partnerships, • Promote individuals’ utilization of community resources and peer supports outside of the facility to promote community integration, Ensuring that clinicians and Behavioral Health Technicians perform the following: • Mediates and advocates on behalf of the individual to help him/her obtain quality services., • Support individuals in identifying their areas of need for treatment services., • Identify Barriers to full participation and develop strategies to overcome them. Ensuring that clinicians perform the following: • Inform Clinical Coordinator of clients who are unavailable for 2 consecutive visits. Job Description The Complaints and Grievances Behavioral Health Liaison will be responsible for the scheduling of Second Level Complaints and Grievances in Accordance with PS&R, Appendix H. Services are delivered in the home or community setting within the Intensive Behavioral Health Services (IBHS) program. Therapy is focused on resiliency factors and connection to community-based supports as identified by the treatment team. The MT utilizes a strengths-based approach to empower children with emotional and behavioral needs to thrive and to help the child and family reach their goals. Mobile therapy services consist of individual therapy, family therapy, development and revision of the ITP (as needed), assistance with crisis stabilization and assistance with addressing problems the child, youth or young adult has encountered. POSITION RESPONSIBILITIES: • Provide evidence-based, skill-based psychotherapy in the home/community environment respectful to the child/family schedule and as prescribed medically necessary services based on clinical need., • Collaborate with other members of the treatment team in order to design and direct the implementation of an individualized treatment plan (ITP) to be implemented in the child’s natural setting(s)., • Develop a trusting partnership with the child, family members and other team members., • Ensure clinical interventions are properly formulated and implemented., • Observe child in the home/school/community setting as part of the initial assessment (as assigned) in preparation for designing the individualized treatment plan and implementation of the written order., • Develop ITP (as designated) to instruct other members of the therapeutic team (including teacher, teacher’s aides, other school personnel, family, and/or other designated individuals providing direct service or supports to the child) in their roles and responsibilities in implementing the established interventions., • Complete and submit accurate and timely documentation which includes but is not limited to: Individualized Treatment Plans (ITP), Encounter Forms, Progress Notes, Progress Summaries (as assigned), Addendums (as assigned), Crisis/Safety Plans, Assessments (as assigned), Structured Tools, Incident Reports, Discharge Summaries (as assigned), etc. by assigned deadlines., • Supervision Requirements:, • MT clinicians who provide services must receive at least one hour of individual face-to-face supervision per month which includes oversight of the following: The interventions being implemented, the child’s, youth’s, or young adult’s progress towards the goals of the ITP, consideration of adjustments needed to the ITP and the staff person’s skill in implementing the interventions in the ITP., • If the individual who provides MT services supervises BHT treatment team members, the MT must receive the following supervision in addition to the above: One hour of face-to-face supervision per month which must include a discussion of the BHT services being provided., • On-Site Supervision Requirement: MT clinicians who provide services must receive 30 minutes of direct observation of the services being provided every 6 months., • MTs must complete at least 40 hours of training within the first year and 20 hours each year thereafter. These training hours must be Department-approved and related to the specific job functions and in accordance with their individual training plan. The training hours will also include mandatory agency and/or funder or state requirements. Job Description Position Summary: The Behavioral Health and Justice Division (BHJD) is committed to working towards reducing criminal justice involvement for Philadelphians with behavioral health challenges, including Serious Mental Illness (SMI). BHJD organizes its work according to the Sequential Intercept Model, identifying opportunities to intervene at various points of criminal justice involvement to deflect and divert individuals away from further penetration of the system and into the behavioral health system. By helping connect individuals to needed treatment and support services that promote stability in the community, BHJD believes that Philadelphians are less likely to be arrested and can achieve health, well-being, and self-determination. BHJD also aligns with Stepping Up and focuses its efforts on lowering the number of Philadelphians with SMI admitted to the Philadelphia Department of Prisons (PDP), reducing their length of stay, connecting them to treatment/supports upon release, and reducing recidivism. The Forensic Support Team (FST) of BHJD was established to meet the unique needs of Philadelphians who are either: 1. Adjudicated as not competent to stand trial on only misdemeanor charges and committed to a State hospital (e.g., Norristown State Hospital) or 2. Adjudicated as non-restorable. The FST works within PDP and Norristown State Hospital (NSH) in order to assist individuals while in jail, when transitioning from jail to NSH, and when reentering the community typically under the court’s jurisdiction. To support this forensic population, the FST helps individuals navigate the Philadelphia judicial and behavioral health systems while also seeking to address each individual’s unique legal, behavioral, medical, physical, cultural, and/or financial needs. The Clinical Assessor becomes an important part of a team comprised of: one (1) FST Supervisor (a licensed psychologist); one (1) FST Coordinator, three (3) Clinical Assessors, and one (1) Post-Doctoral Psychology Resident. The Clinical Assessor will complete clinical assessments and provide system navigation for justice-involved individuals with behavioral health challenges who fall into either of the aforementioned priority groups. In this capacity, the Clinical Assessor is responsible for determining the appropriate level of care (LOC) for the individual via a procedure that is consistent with a strengths-based, biopsychosocial recovery model. The procedure relies on State hospitals and jails to provide access to the individual as well as clear documentation and consultation about the individual so an informed decision can be made about the appropriate LOC and the individual’s risk for violence. The decision about the LOC is based on the individual’s: aspirations, goals for recovery, strengths, support system, risk for future violence, psychiatric diagnosis, medical diagnosis, psychiatric history, cognitive or physical limitations, results from any psychological measures or medical tests, current medications, incident reports, participation in recovery services, current treatment team input, and privileges earned. With this information and documentation, the Clinician Assessor collaborates with the Philadelphian and their treatment team in an effort to agree on an appropriate LOC as well as a re-entry plan and, when necessary, present the plan to the appropriate criminal court. Duties and Responsibilities: The Clinical Assessor helps individuals with SMI "navigate" the maze of treatment programs, the court system, and support services upon reentry into the community. The Clinical Assessor reduces barriers that keep individuals with SMI from accessing timely treatment by identifying behavioral health needs and directing individuals to appropriate resources. An effective Clinical Assessor is a: Relationship builder Building effective working relationships is the foundation of effective forensic navigation. To improve coordination of care for people with mental illness and to reduce barriers to reentry, navigators must work with different groups of people: individuals who are incarcerated, healthcare teams within the PDP, criminal justice partners, families, NSH staff, and resource providers within the community. Specific duties • Perform level of care and other-discharge related assessments for individuals adjudicated not competent to stand trial and/or non-restorable that are detained at the PDP or NSH, • Attend treatment team meetings for individuals detained at NSH to monitor progress and/or changes in clinical presentation, • Facilitate access to re-entry services and programs, • Identify individuals for diversion and share with appropriate justice partners, • Review legal status, commitment type, charges, competency opinions and findings, and clinical needs for all individuals on the waitlist to NSH on an ongoing basis, • Maintain accurate, confidential records documenting services provided and unmet needs using appropriate databases, • Remain current with resource and referral information, • Maintain list of community resources, • Build and maintain relationships with key individuals (e.g., PDP staff, Public Defender), and provide updates upon marked changes in an individual’s clinical or legal status to trigger appropriate interventions, • Coordinate with courts to ensure timely court hearings and legal proceedings, particularly when an individual’s clinical or legal status changes, • Provide behavioral health literacy and continuity of care training to both behavioral health providers and criminal justice personnel, • Provide updated progress reports for criminal court hearings and attend those hearings when necessary This position requires a master’s level degree in social work, psychology, or a related clinical focused field with at least two years of experience working with individuals with serious mental illness and/or in clinical-forensic assessment. Job Description The Clinical Care Manager (CCM) Specialist- Children’s Complex Care (High-Fidelity Wraparound) is part of a specialized team, responsible for providing high intensity care management, that works collaboratively and directly with members and providers to ensure effective, holistic, member-centric care.
