Job Description JOB SUMMARY: Accountable for the observation, assessment, intervention, evaluation, rehabilitation, or health teachings of a person who is ill, injured, infirm, or experiencing a change in normal health processes, the maintenance of health or prevention of illness; the administration of a medication or treatment as ordered by a physician, the supervision or teaching of nursing; the administration, supervision, and evaluation of nursing practices, policies, and procedures; the performance of an act delegated by a physician and the development of the nursing care plan. Also, administers hyperbaric oxygen therapy and diagnostics testing to patients under the supervision of a hyperbaric patient physician, applicable when complete HBO certification training. Shift: Days PRINCIPAL DUTIES AND RESPONSIBILITIES: Service: • Consistently supports and communicates the Mission, Vision, and Values of St. Joseph Medical Center., • Upholds the Standards of Conduct and Corporate Compliance., • Identify patients care situations that require intervention, implementimplement nursing actions, and evaluate responses. Identify patient and family education and discharges needs to implement and document a teaching plan of care., • Implement wound care and HBO guidelines when clinically indicated. Assist physician with wound therapy and procedures., • Coordinate activities with other disciplines, may include rehab, nutrition, diabetes education, home health, durable medical equipment (DME), nursing homes and other ancillary services., • Other duties as assigned., • Perform blood glucose monitoring as ordered and is indicated by patient diagnosis., • Perform dressing changes and/or wound care within scope of practice, as ordered. Take serial photographs of the wound for reporting the patient's progress., • Perform an inventory check, maintain a record of supplies, and laundry items. Maintain equipment checks and backup air and oxygen supplies on daily basis., • Incorporates the patient's physiological, psychological, cultural, developmental and spiritual needs into the plan of care., • Consistently follows patient care, and facility guidelines and procedures in performance., • Directs support personnel in implementing patient's plan of care, provides supervision and guidance, and evaluates effectiveness of the delegated tasks., • Collaborates with inter and interprofessional colleagues to ensure quality outcomes., • Assist/collaborates with caregivers for patient's skin problems such as draining wounds, fistulas, pressure ulcer, incontinence care, etc. Selects and recommends ostomy or wound management system products and equipment to patient's family and organization., • Follow-up with patients who miss an appointment to reschedule., • Completes annual education requirements., • Maintains competency, as evidenced by completion of competency validation requirements., • Promotes a culture of patient safety which results in the identification and reduction of unsafe practices., • Recognize and assess initial alterations in the patient's body systems and record assessment data in an orderly fashion and communicate, revise and verbalize pertinent information to other health care team members., • Assess, stage, and measure wounds accurately and in accordance with guidelines. Photograph wounds according to guidelines., • Document a plan of care accurately for all assigned patients., • Formulate desired outcomes that are specific to patient's identified issues and AWCC guidelines. Formulate desired outcomes that are specific and measurable within a certain time frame and consistent with other health provider's exceptions., • Other duties as assigned. Applicable for HBO certified RNs: Ascertain that the patient's diagnosis is one of the approved conditions for Hyperbaric Oxygen Therapy, and that the said diagnosis is clearly entered on the patient's charts/records., • Implement the physician's orders as indicated by the patient's treatment plan., • Evaluate the patient outcome and meet with the Manager/Director and the hyperbaric physician to discuss patient treatment and adjust treatment as ordered by physician and as necessary. Consult with hyperbaric physician regarding any adverse reactions., • Document treatment notes and maintain patient treatment/diagnostic records. WORK EXPERIENCE: · Minimum of 2 years medical-surgical experience. Wound care experience preferred. Bilingual preferred. LICENSE/REGISTRATION/CERTIFICATION: Licensed in State of Texas as a Registered Nurse required. Preferred certification in wound care. Current American Heart Association Basic Life Support (BLS) certification is required. EDUCATION & TRAINING: Registered Nurse, graduate from an accredited School of Nursing, Bachelor of Nursing preferred. Courses in wound management ostomy management or certification as a wound ostomy care nurse (WOCN) preferred. SKILLS: Wound care and management skills for patients with decubitus ulcer, peripheral and stoma ulcer, fistulas, etc. Advanced knowledge of sterile dressing technique and wound management product use. Demonstrate professionalism and commitment to the mission statement of AWCC as well as St. Joseph Medical Center at all times. Technical, critical thinking, and interpersonal skills relevant to area in order to effectively communicate and work well with providers, staff members, administrative, patients, family members, community members, and support staff in a dynamic and fast paced practice while willing to work under pressure. Ability to prioritize work with minimal supervision, in order to independently carry out the duties of the position. Able to communicate effectively, in English. Additional languages preferred. Computer knowledge including Word, Excel, Power Point, and Outlook. Maintain patient confidentiality at all times. Computer literacy required. Houston's oldest hospital is GROWING!
