POSITION OVERVIEW The Case Manager provides supports the wellness and recovery goals of individuals with complex and/or chronic behavioral health and substance abuse issues and needs by implementing targeted interventions designed to provide timely, high-quality, and efficient care. Targeted Case Management services are tailored around goals aimed at providing services that encourage individuals to resolve problems that interfere with their attainment or maintenance of independence or self-sufficiency. The Case manager must have knowledge of community resources and counseling/social work practices with high-risk populations. The Case Manager must have experience working with persons in crisis while attaining the ability to work independently with a strong sense of focus. Must be task-oriented, nonjudgmental and maintain boundary with individuals. The case manager must be able to work in a variety of settings with culturally diverse families while having the ability to be culturally sensitive and appropriate. Maintain a caseload and meet with patients on a weekly basis. The goal of the case manager is to help patients regain optimum health or improve functional capability, in the right setting. It involves assessments, comprehensive diagnostic and treatment planning evaluation to achieve wellness and recovery goals. Case managers are the cohort in accessing services via referral, linkage to needed services, and monitoring follow up on patient care in order to meet the individual’s needs. KEY ESSENTIAL FUNCTIONS • Provide targeted case management services to children, youth and adult consumers living with mental health illnesses and substance abuse disorders and their families/support systems through ensuring access to care, engagement in care coordination of care to obtain the full range of needed services., • Gather enrollment consents, PSYCKES/RHIO consents, and complete screening, baseline-risk assessments, reassessments, plan of care, plan of care updates and notes in accordance with departmental policies., • Demonstrate the ability to clearly articulate, verbally and in writing, the aims and goals of the department to potential patients, community members and staff., • Participate in quality improvement activities, projects and reviews in collaboration clinical team members., • Complete daily, weekly, monthly, or other periodic requests for narrative or quantitative data reports for program review., • Prioritize the homeless population through identification of new sources of potential patients, onsite meetings with patients at their shelter and conduct outreach and engagement presentations., • Meet regularly with supervisor and attend staff meetings and case conferences. Be prepared to discuss case management and operational issues impacting performance and program operations., • Complete and submit daily activity log in accordance with departmental policies., • Ensure patient is attending scheduled medical and social service visits through building relationships with patients and providers. Coordinate and schedule appointments with Social Worker and Medical/Mental Health providers. Routine calls should be made to internal and external providers before and after visits to follow up and provide necessary support to the patients., • Maintain four contacts with each client or at a greater frequency as indicated by the risk stratification and plan of care., • Access and respond per agency guidelines to client complaints of grievances, • Conduct outreach and engagement in accordance with policies via phone, electronic methods, and letter and or field work to client/collateral/provider to engage clients or strengthen connectivity., • Help maintain health and wellness and prevent secondary disease complications through provision of health information, support plan of care, and coaching., • Promote and expand linkage development in all areas related to patient care including social determinants (e.g. housing, employment) and monitor the effectiveness of linkages with other service providers via phone, face to face meetings of formal case conferences., • Communicate effectively with Supervisor in identifying strengths, weakness and opportunities of program operations., • Ensure community-follow up to engage the client in care; promote compliance with medical appointments and encourages client self-sufficiency and empowerment., • High School Diploma or GED required., • Associate Degree and Bachelor’s degree preferred., • Minimum of six (6) years of experience in case management., • Must obtain Mandated reporter (2 hours) training/certificate required., • Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate within three (3) months of hire date., • Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate, • Must be trained in Trauma Informed Care and in Military and Veteran’s culture, • Must be team oriented and possess a positive attitude with a willingness to be flexible and helpful., • Must be able to multi-task with strong organizational skills., • Excellent interpersonal skills and able to communicate both verbally and in written form., • Commitment to cultural diversity and sensitivity. BronxWorks is seeking a highly motivated Homeless Prevention Case Manager to join our Homebase team. As a case manager, you will play a critical role in helping individuals and families at risk of homelessness stay stably housed or relocate to stable housing. Your responsibilities will include screening and assessing client needs and developing strategic interventions to stabilize clients' housing. You will be expected to engage clients, assess for program and/or subsidy eligibility, develop service plans, conduct Housing Court advocacy, document progress, complete virtual/in person unit inspections, and follow up with external parties. You will connect clients with resources like rental arrears assistance, public benefits and housing vouchers. You will manage a caseload of approximately 40-45 clients in various stages of a housing crisis. POSITION OVERVIEW Under supervision of the Program Director the Clinical Supervisor will provide clinical oversight and quality assurance to all unlicensed and licensed staff clinicians who provide substance and alcohol abuse services in an OTP program setting. The Clinical Supervisor is responsible for adherence to all applicable licensing, supervision, billing, documentation requirements and overseeing the timeliness and quality of a staff member’s clinical work. The Clinical Supervisor is responsible for client case record reviews, performance improvement and quality improvement to include all required monthly reports. KEY ESSENTIAL FUNCTIONS • Responsible for the daily operations and performance of the OTP program, • Manage OTP clinic operations to budgeted and planned results., • Work closely with staff via weekly supervision to ensure the completion of performance goals. Provides ongoing, regular feedback and performance evaluation to clinicians on the quality and timeliness of their clinical work as well as their caseload. Also solicits feedback from clinicians regarding supervision and whether needs are being met., • Monitor caseloads and ensure the supervisees comply with all agency, clinical, legal and ethical practices. Review and co-sign supervisees’ clinical documentation to include but not limited to methadone dose change request forms, bio-psychosocial history, initial treatment plans and treatment plans reviews., • Becomes familiar with APG and ICD-10 billing codes to include DSM V diagnosis codes., • Oversight and monitoring of billable hours of service required. Provide support, training, and remediation where needed to assist supervisees in meeting program’s billing expectations and caseload requirements, charting, recordkeeping, documentation standards and all other quality assurance standards., • Communicate any concerns about quality of clinician’s work to Program Director in a timely manner., • Become familiar with ACACIA policies and procedures and implements in OTP program. Ensures all administrative and clinical policies and procedures are implemented appropriately and effectively. Also ensures all medical and other services to which patients may be referred are known to staff and made available to patients., • Communicate with and ensures compliance with regulatory bodies (OASAS, Joint Commission, SAMHSA, DEA, HIPPA) to include state and federal regulations., • Assist with partnering opportunities, linkage/support agreements and other critical relationships to drive census growth and meet regulatory requirements., • Work closely with Medical Director, Program Director, and Operations Director in a variety of areas including but not limited to: compliance with federal and state rules; laws and regulations; compliance with Joint Commission, SAMHSA, OASAS and DEA; performance improvement initiatives; measure of success; productivity goals; quality client care; customer service; operational enhancement; growth of client census; contractual requirements; standardization and conformity; best practices and plans for continuous improvement, • Become familiar with Joint Commission and OTP federal and state laws, regulations, etc. and the applications of such., • Participate in community relations and development activities to drive and maintain census., • Identify and implement tactical steps to increase and retain census., • Work with clinic team to ensure operations are prepared to handle increased census., • Identify training needs and makes recommendations to the Program Director. The Social Service Supervisor will provide supervision to Case Managers using Evidence Based Practices (EBP), Motivational Interviewing (MI), Trauma Informed Care (TIC), and boundary setting. Responsibilities: * Provide comprehensive case management including individual and group counseling services and other appropriate support services to clients in the absence of the Case Managers. * Supervise Case Managers and provide coaching, training, and staff development as needed. * Manage the shelter’s multidisciplinary team. * Champion the use of Evidence Based Practices (EBP) in all aspects of Win services. * Use and promote EBP, MI, MIC, and TIC to exceed program goals. * Assume responsibility for Program Services in the absence of the Program Director. * Participate in DHS daily housing metrics calls. * Provide crisis intervention, oversee day-to-day case management services and ensure weekly case conferences. * Ensure all Case Managers conduct an initial needs assessment with new families within 48 hours of arrival at the shelter and begin to develop an Individualized Living Plan (ILP). * Participate in Child Safety Conferences and provide information as a Mandated Reporter to the Administration for Children’s Services (ACS) as needed. * Ensure Case Managers are provided with orientation and training related to their responsibilities in maintaining the Uniform Client Chart. * Ensure Case Managers maintain current and accurate confidential case records with all required documentation. * Audit the Client Assistance and Rehousing Enterprise System (CARES) database, review essential documentation, and monitor chart maintenance for OTDA, DHS, and Win audit readiness. * Work with community service providers to ensure clients receive essential services including mandated substance abuse treatment and mental health services. * Attend regularly scheduled CARES training sessions to obtain DHS updates. * Develop and conduct client activities/workshops. * Schedule and attend client monthly house meetings. * Provide feedback to the Program Director to ensure resources are available and the goals of the program are achieved. * Prepare accurate and timely reports required by funders and Win. * Perform other related duties as needed. Essential Functions: * Must be able to work one evening weekly, one Saturday a month, and holidays. * Must be able to ascend/descend stairs in the building where client families reside. * Must be able to escort clients to appointments as needed. Qualifications: * Commitment to Win’s mission, vision and values. * M.S.W. or a related degree required with 3 to 5 years of experience in family and/or single adult residential counseling services. * At least one year of supervisory experience a plus. * Familiarity with EBP, MI, and TIC. * Must be able to work with at-risk populations including domestic violence survivors, clients with medical and mental health issues, and the chronically homelessness. * Must be able to work effectively with a diverse staff. * Excellent organizational and communication skills, written and verbal. * Must be able to work effectively in a team environment * Familiarity with entitlement systems and procedures. * Working knowledge of substance abuse issues a plus. POSITION OVERVIEW The Credentialed Alcoholism and Substance Abuse Counselor (CASAC) provide alcoholism and substance abuse counseling services in approved work settings. The CASAC provides comprehensive assessment services to clients who may have substance abuse barriers to employment by applying clinical judgment to determine appropriate treatment needs and levels of care. This includes performing diagnostic assessment, evaluation, intervention, referral and alcoholism and/or substance abuse counseling in both individual and group settings. KEY ESSENTIAL FUNCTIONS • Carries an active caseloads of clients as stipulated by the program, • Provides 28 hours of direct care services each week, • Orients clients about program rules and regulations, goals and objectives, client’s obligations, client’s rights and programs operation, • Complete clinical assessment related paperwork upon admission, • Refers to mental health for assessment upon admission. Assessment is repeated as needed., • Complete daily progress notes and encounters for billing on time, • Engage client in the creating of Recovery Treatment Plan as evidenced by submitting plan with all signatures. Plan is developed according to OASAS regulations., • Documents all individual, family, groups, and or crisis interventions counseling plus any other significant contact in the client’s clinical record., • Documents techniques used during individual, family counseling sessions and group therapy to address behaviors, attitudes and feelings and appropriately assesses their effects., • Facilitate groups weekly. Documentation is completed on time., • Provide regular ongoing direct services to clients, their families/significant others, helping them with adjustments problems, the need for placement, the diagnosis and implications, substance abuse counseling, etc., • Maintain ongoing cooperative relationship with other disciplines to assure quality service to clients., • Assist client in locating housing and applying for permanent housing as needed., • Maintain and updates referral service and resources that may be useful to our population, • Submit required chart documentation adhering to OASAS regulatory timeframes, • High School Diploma required., • Associate/Bachelor Degree preferred, • Minimum of three (3) years’ experience in a Mental Health setting., • Valid unrestricted OASAS certification as a CASAC., • Must have specific training in chemical use, abuse and dependence specific to the services provided., • Must obtain Mandated reporter (2 hours) training/certificate prior to hire date., • Obtain within six months of hire:, • Supporting Recovery with Medications Addictions Treatment (MAT), • Obtain within thirty days of hire:, • Screening, Brief Intervention and Referral to Treatment, • CASAC Canon of Ethics (6 hours), • Confidentiality related to 42CFR (3 Hours) Acacia Network provides a comprehensive and competitive benefits package to our employees.
